The aim of this study was to verify the prevalence of metabolically healthy obese (MHO) phenotype and associated factors in South American adolescents who are overweight. ⋯ The prevalence of MHO in South American overweight adolescents was high and varied according to the definition used. Age, sex, WC, and triacylglycerolslevel were independent predictors of the MHO phenotype in this population.

The role of ADIPOQ gene variants on weight loss after a dietary intervention remain unclear. The aim of this study was to analyze the effects of rs266729 of the ADIPOQ gene on cardiovascular risk factors and adiposity parameters after adherence to a Mediterranean-type hypocaloric diet. ⋯ After weight loss, the CC genotype of ADIPOQ gene variant (rs266729) is associated with increases in adiponectin levels and decreases of low-density lipoprotein cholesterol, insulin and homeostasis model assessment for insulin resistance after weight loss.

The aim of this study was to evaluate kidney function outcome in adults on home parenteral nutrition (HPN) for chronic intestinal failure using the newly recommended equations for estimated glomerular filtration rate (eGFR) assessment in clinical practice. ⋯ In patients on HPN for chronic intestinal failure, decreased kidney function is a frequent finding, even at HPN commencement, demanding accurate monitoring during the treatment. Prevention of CKD primarily relies on the maintenance of fluid balance and the prevention of catheter-sepsis and urologic diseases.

Cardiovascular disease (CVD) is the leading cause of mortality globally. Studies have suggested that supplementary ω-3 oils may provide cardiovascular protection, although the literature is equivocal. Recently, it has been established that many commercially available ω-3 supplements are unacceptably oxidized, leading to myriad potential health risks. One oxidation product of concern is aldehydes, which have been shown to have mutagenic, cytotoxic, and inflammatory properties that may contribute to many different disease processes, including CVD. The aim of this study was to assess the prevalence of aldehyde contamination in commercially available ω-3 supplements. ⋯ The four krill products contained aldehydes at concentrations between 5.652 (±0.496) and 6.779 (±1.817) mMol/L. Both algae samples contained aldehydes: 1.235 (±0.111) and 1.565 (±0.618) mMol/L. Two of the six fish oils contained aldehydes 1.568 (±0.291) and 4.319 (±2.361) mMol/L. There is currently no standard for aldehyde content nor for labeling of ω-3 supplements. Two-thirds (8 of 12) of the ω-3 supplements tested in this study contained aldehydes. Aldehydes have the potential to precipitate serious health problems even at very low absolute intake volumes. These findings may provide reason for sober reflection.

Strategies for providing optimal nutritional therapy have evolved over time, with the emphasis on specific directives (such as route, use of immunonutrition, high protein, organ-specific formulas, etc.), achieving variable degrees of success for improving outcomes in the intensive care unit. As the largest immune organ in the body comprising the largest interface between the host and the external environment, the gut can have an amplifying effect on a pattern of dysbiosis, immune dysregulation, and multiple organ failure seen in the critically ill patient. ⋯ Simply providing refined polymeric formulas as enteral nutrition may not take full advantage of the potential for optimal outcome that could be derived by giving therapy designed to directly stimulate gut defenses and support the intestinal microbiota. This article describes a series of strategies (such as use of intact whole food formulas, soluble fiber, fecal microbial transplantation, serum bovine immunoglobulin, or agents to promote commensal behavior) that should modulate the gut microbiome and shift the critically ill patient toward a pattern of health and recovery.
