To survey anesthesiologists to assess medication injection safety knowledge and practices, and to improve infection control programs of the New York City Department of Health and Mental Hygiene and the New York State Society of Anesthesiologists (NYSSA). ⋯ Although they were not necessarily representative of all anesthesiologists in New York State, unacceptable or high-risk injection practices were common among respondents despite widespread knowledge regarding injection-contamination mechanisms. System barriers contribute to the use of medication vials for multiple patients.

In 2010, the American Board of Anesthesiology instituted a new Maintenance of Certification in Anesthesiology (MOCA) Part IV activity requiring diplomates to attend and self-reflect on a simulation-based course in an American Society of Anesthesiologists-endorsed program. Although there are certain course requirements, much of the curriculum and structure of these MOCA activities is left to the discretion of the participating endorsed program. The ideal course would emphasize multimodality simulation-based activities that optimize diplomate education and satisfaction, while economizing faculty requirements. We describe of our course structure and content as a potentially useful template.

To evaluate the Warmflo fluid warming system at various flow rates of crystalloid solution simulated to clinical conditions. ⋯ The Warmflo fluid warming system can overheat fluids at temperatures considerably above normal body temperatures.

A 14 month old, 19 kg female infant with aminotic band syndrome and severe craniofacial malformations presented for placement of an open gastrostomy tube. Spontaneous ventilation was maintained while an "awake look" was performed with the video laryngoscope using intravenous propofol (30 mg total) for sedation. A Grade I Cormack-Lehane score was obtained on the monitor. Succinylcholine was then administered intravenously and orotracheal intubation was performed on the first attempt with a size 4.5 endotracheal tube.
