Randomized Controlled Trial Clinical TrialComparative effect of 6% hydroxyethyl starch (containing 1% dextrose) and lactated Ringer's solution for cesarean section under spinal anesthesia.
This study aimed to compare low-molecular weight hydroxyethyl starch containing 1% dextrose (HES) infusion and lactated Ringer's solution (LR) in the prevention of hypotension associated with spinal anesthesia for cesarean section. ⋯ This study did not show an advantage of HES compared with LR in the prevention of hypotension or in the reduction of ephedrine dose during cesarean section under spinal anesthesia. The anesthesia level, rather than the choice of intravenous fluid solution, might be related to the ephedrine dose.

Effects of fentanyl on cardiovascular and plasma catecholamine responses in surgical patients.
Whether opioids administered before skin incision, under inhalational anesthesia, improve cardiovascular and plasma catecholamine responses to surgical stimulation compared with those administered after skin incision remains unclear. We compared the effects of fentanyl injected before and after skin incision on these responses. ⋯ Our results indicated that fentanyl depressed cardiovascular and plasma catecholamine responses irrespective of the time of administration, and that the higher dose of fentanyl produced a greater suppression of MAP and HR responses. In addition, the depressant effects on MAP of high-dose fentanyl administered 5 min before skin incision lasted longer than when injected 5 min after incision. At both doses, the opioid attenuated the rise in plasma Epi, but not Nor.

Randomized Controlled Trial Clinical TrialThe effect of propofol infusion on minimum alveolar concentration of sevoflurane for smooth tracheal intubation.
This study was conducted to determine the effect of propofol infusion on the minimum alveolar concentration necessary for smooth tracheal intubation (MACEI) of sevoflurane. ⋯ Propofol infusion reduced sevoflurane MACEI. This finding suggests that propofol would be an excellent adjuvant to use with sevoflurane for tracheal intubation.

Prevention and treatment of malignant hyperthermia in certified training hospitals in Japan: a questionnaire.
To assess the preparedness of hospitals in Japan for cases of malignant hyperthermia (MH). ⋯ The results of the survey revealed that some hospitals had inadequate monitoring methods and a lack of prepared dantrolene for cases of MH under general anesthesia. We recommend that essential monitors be deployed and adequate preparations of dantrolene be maintained for effective early diagnosis and treatment of MH.

Suppressive effect of spinal dorsal-horn neuronal activity by local spinal-cord cooling is reversed by naloxone in cats.
The purpose of this study was to assess the effect of local spinal cord cooling on spinal dorsal-horn neuronal activity, with special emphasis on the role of endogenous opioid. ⋯ The results demonstrate that endogenous opioids may play an important role in dorsal-horn neuronal suppression induced by local spinal-cord cooling.
