Randomized Controlled TrialAn individualized recruitment maneuver for mechanically ventilated patients after cardiac surgery.
The recruitment maneuver (RM) has been shown to improve oxygenation for post-cardiopulmonary bypass (CPB) patients; however, sustained inflation of the lung gives rise to hypotension. The primary goal of our study was to evaluate the safety and efficacy of our proposed RM, defined on the basis of dynamic lung compliance (Cdyn). ⋯ The present study indicates that the individualized RM resulted in minimum changes of hemodynamics and brought about improvement in oxygenation and lung compliance.

Randomized Controlled TrialEffect of landiolol hydrochloride on suxamethonium-induced neuromuscular block.
The aim of this study was to examine the effect of landiolol hydrochloride, an ultrashort-acting beta(1)-blocker, on suxamethonium-induced neuromuscular block. ⋯ Landiolol delayed recovery from suxamethonium-induced paralysis. However, the interaction between the drugs seemed to be small in the clinical setting.

Randomized Controlled TrialLow dose of fentanyl reduces predicted effect-site concentration of propofol for flexible laryngeal mask airway insertion.
In contrast to reports on the classical laryngeal mask airway (classical LMA; CLMA), no report has calculated the 50% and 95% effect-site concentrations (EC(50) and EC(95), respectively) of propofol required for flexible LMA (FLMA) insertion. This study was designed to determine the EC(50) and EC(95) of propofol for FLMA insertion, using probit analysis, and to investigate whether supplemental 0.25 microg x kg(-1) fentanyl decreased these concentrations. ⋯ The propofol EC(50) for FLMA insertion was decreased by supplemental 0.25 microg x kg(-1) fentanyl without BIS, hemodynamic, or respiratory depression.

Randomized Controlled Trial Comparative StudyThe comparative effects of sevoflurane versus isoflurane on cerebrovascular carbon dioxide reactivity in patients with previous stroke.
The use of volatile anesthetics is reportedly related to altered cerebrovascular carbon dioxide (CO2) reactivity. We examined the comparative effects of sevoflurane versus isoflurane on cerebrovascular CO2 reactivity in patients with previous stroke. ⋯ Our findings suggest that, in patients with previous stroke, cerebrovascular CO2 reactivity under sevoflurane anesthesia was lower than that under isoflurane anesthesia.

Randomized Controlled TrialAdding intrathecal morphine to unilateral spinal anesthesia results in better pain relief following knee arthroscopy.
Intrathecal morphine is administered to provide profound and prolonged analgesia, and to treat acute postoperative pain. We compared the effectiveness of hyperbaric bupivacaine alone and in combination with morphine for unilateral spinal anesthesia in patients undergoing knee arthroscopy. ⋯ We conclude that unilateral spinal anesthesia with hyperbaric bupivacaine plus 0.16 mg morphine is preferable to hyperbaric bupivacaine alone with respect to analgesic requirement, duration of analgesia, and VAS values.
