ReviewTen-year results of tibial osteotomy for medial gonarthrosis. The influence of overcorrection.
The results in relation to the correction achieved 1 year after surgery of a series of tibial osteotomies in 50 patients (52 knees) are presented with a total observation time of 10 years. Mean age at the time of osteotomy was 56 years. ⋯ One out of 34 overcorrected knees recurred in varus. Five knees were revised by knee arthroplasty or reosteotomy one of which was overcorrected.

Nonunion is a very rare complication of a comminuted intra-articular wrist fracture treated by external fixation. The authors describe the likely reasons and the after-care, in order to avoid this complication.

ReviewSystemic effects of implanted prostheses made of cobalt-chromium alloys.
Systemic effects of Co-Cr alloy/polyethylene hip joint prostheses were investigated using instrumental neutron activation to determine the concentrations of up to 16 elements. First, in a prospective study whole blood and serum taken from 10 patients from 1 day before to 90 days after implantation were analyzed. Secondly, in a retrospective study whole blood and serum from 23 patients who had had prostheses in place for up to 18 years were analyzed. ⋯ The analyses of tissues from the vicinity of the implants in the two deceased implant patients showed strong local effects of Co, Cr, Zr, and Hf, the latter two elements originating from the X-ray contrast media in the bone cement. The analyses of organs revealed significant Co and Cr enrichment in several tissues and organs. Consequently, it can be seen that implant corrosion is not an occurrence of merely local significance, but one that affects the trace element status of the entire organism.

Randomized Controlled Trial Clinical TrialEffect of high-dose corticosteroids on the incidence of deep vein thrombosis after total hip replacement.
Fifty patients receiving uncemented total hip prostheses were examined by venography of the legs on the 2nd postoperative day. The patients were randomly divided into two groups, a non-steroid group (n = 26) and a steroid group (n = 24). Both groups received dextran thrombo-prophylaxis. ⋯ All patients were followed up clinically and plethysmographically up to 12 months after surgery. Distally located asymptomatic DVT were not given specific treatment. The postoperative course was uneventful except for one patient in each group who developed clinically, apparent DVT more than 3 weeks after operation, although the initial venographic studies were normal.

A prospective study of repair of longitudinal, peripheral, and posterior horn meniscus tears was carried out in 50 patients. The mean length of follow-up was 5 year. Forty-eight of the patients were symptom-free and post-repair arthroscopic examination confirmed that the repaired menisci have healed satisfactorily. The rationale and technique for surgical repair of the meniscus of the knee are presented.
