Evaluation of the interaction between contact force and decision making on lower extremity biomechanics during a side-cutting maneuver.
The purposes of this study were (1) to compare the effect between contact force [first and second ground reaction force (GRF)] and decision making (anticipation vs unanticipation) on lower extremity biomechanics during a side-cutting maneuver in young soccer players; and (2) to identify which condition is more vulnerable to biomechanical risk factors of the anterior cruciate ligament (ACL) injury. ⋯ The GRF and muscle activities were smaller in the first peak than those in the second peak; however, first peak showed a closer association with biomechanical risk factors of the ACL injury. There were some interactions between contact force and decision making and unanticipation made the first peak more risky to the ACL injury.

Surgeon preferences regarding antibiotic prophylaxis for ballistic fractures.
Scant evidence exists to support antibiotic use for low velocity ballistic fractures (LVBF). We therefore sought to define current practice patterns. We hypothesized that most surgeons prescribe antibiotics for LVBF, prescribing is not driven by institutional protocols, and that decisions are based on protocols utilized for blunt trauma. ⋯ Routine antibiotic use for LVBF is common; however, practice is not dictated by institutional protocol. Although antibiotic use generally follows current blunt trauma guidelines, surgeons do not base their treatment decisions the Gustilo-Anderson classification. Given the high rate of antibiotic use for LVBF, further study should focus on providing evidence-based treatment guidelines.

Prevention of cement leakage into the hip joint by a standard cement plug during PFN-A cement augmentation: a technical note.
Medial penetration of the helical blade into the hip joint after fixation of trochanteric fractures using the proximal femur nail antirotation (PFN-A) is a potential failure mode. In low demand patients a blade exchange with cement augmentation may be an option if conversion to total hip arthroplasty is unfeasible to salvage the cut-through. This article describes a technique to avoid intraarticular cement leakage using a cement plug to close the defect in the femoral head caused by the cut-through.

ReviewBiomechanical effect of unloader braces for medial osteoarthritis of the knee: a systematic review (CRD 42015026136).
There is a lack of consensus regarding biomechanical effects of unloader braces for the treatment of medial osteoarthritis (OA) of the knee. The purpose of this study was to perform a systematic review of studies examining the biomechanical effect of unloader braces. ⋯ This systematic review could demonstrate evidence that unloader braces reduce the adduction moment of the knee. Foresighted, a systematic review about the clinical effect of unloader braces is required.

Randomized Controlled TrialMeasuring long radiographs affects the positioning of femoral components in total knee arthroplasty: a randomized controlled trial.
The aim of this study was to investigate if preoperative measurements of the femoral valgus angle (FVA) affected the mechanical alignment, individual component positions and clinical outcome in total knee arthroplasty (TKA). ⋯ Preoperative FVA measurement and following femoral distal cut adjustments did not affect overall leg alignment postoperatively, while positioning of femoral component was improved together with minor improvements in objective KSS subscale scores.
