Randomized Controlled Trial Multicenter StudyCervical pessaries to prevent preterm birth in women with a multiple pregnancy: a per-protocol analysis of a randomized clinical trial.
We recently showed that a cervical pessary prevents preterm birth and reduces poor neonatal outcomes in women with a twin pregnancy and a short cervix (<38 mm). The objective of this study was to evaluate the full potential treatment effect of the pessary in the whole group and in women with a short cervix. ⋯ The analysis confirms the principal findings of the intention-to-treat analysis. Time to delivery was longer in the pessary group than in the control group when censored data were used. This implies the pessary should not be removed until labor is evident.

Multicenter Study Observational StudyCan we improve the prediction of pouch of Douglas obliteration in women with suspected endometriosis using ultrasound-based models? A multicenter prospective observational study.
A negative "sliding sign" during transvaginal sonography (TVS) is associated with pouch of Douglas (POD) obliteration at laparoscopy in women with suspected endometriosis. The aim of the current study was to develop and validate mathematical ultrasound models to determine whether a combination of TVS markers could improve the prediction of POD obliteration as compared with the TVS "sliding sign" alone. ⋯ Incorporation of TVS markers such as bowel endometriosis, endometrioma and ovarian fixation into mathematical models does not appear to improve the prediction of POD obliteration as compared with the TVS "sliding sign" alone.

Multicenter StudyEmergency peripartum hysterectomy: results from the prospective Nordic Obstetric Surveillance Study (NOSS).
To assess the prevalence and risk factors of emergency peripartum hysterectomy. ⋯ A combination of prospective data collected from clinicians and information gathered from register-based databases can yield valuable data, improving the registration accuracy for rare, near-miss cases. However, proper and uniform clinical guidelines for the use of well-defined international diagnostic codes are still needed.

Multicenter StudyThe Nordic Obstetric Surveillance Study: a study of complete uterine rupture, abnormally invasive placenta, peripartum hysterectomy, and severe blood loss at delivery.
To assess the rates and characteristics of women with complete uterine rupture, abnormally invasive placenta, peripartum hysterectomy, and severe blood loss at delivery in the Nordic countries. ⋯ The studied obstetric complications are rare. Uniform definitions and valid reporting are essential for international comparisons. The main risk factors include previous cesarean section. The detailed information collected in the NOSS database provides a basis for epidemiologic studies, audits, and educational activities.

Randomized Controlled Trial Multicenter StudySingle-dose tranexamic acid in advanced ovarian cancer surgery reduces blood loss and transfusions: double-blind placebo-controlled randomized multicenter study.
To determine whether single-dose tranexamic acid given intravenously immediately before surgery for presumed advanced ovarian cancer reduces perioperative blood loss and blood transfusions. ⋯ A single dose of tranexamic acid given immediately before surgery reduces blood loss and transfusion rates significantly in advanced ovarian cancer surgery. Tranexamic acid may be recommended as standard prophylactic treatment in advanced ovarian cancer surgery.
