MRI appearance of subthalamic nucleus (STN) boundaries in Parkinson's patients is often unreliable and not well understood. An objective comparison between FSE T2 and inversion recovery (FSTIR) sequences for stereotactic placement of deep brain stimulators is presented to advance current understanding of STN tissue contrast for refractory Parkinson's disease (PD). ⋯ STN, a common stimulation target, shows an age dependent trend for normalized FSTIR MRI contrast. Although larger patient pools are needed, our work points to tissue relaxation-based changes in STN that may provide insight in early stages of brain pathology involving DBS targets in medically refractory Parkinson's disease.

MRI studies in Parkinson's Disease have shown volumetric reductions of subcortical structures such as the thalamus, putamen, globus pallidus, and caudate nucleus. However, there are no studies which look at the relationship between subcortical structure volumes and clinical variables, such as age and motor severity scores. ⋯ There is a correlation between the volume subcortical structures and clinical variables, particularly the disease duration, in PD. This may not only help understanding the disease process but also patient selection for invasive and noninvasive therapies.

Multipurpose ultrasound probes combined with ultra-mobile ultrasound instrumentation have the potential to increase the availability and use of ultrasound examinations in the assessment of atherosclerotic burden and cardiac disease. The aim of this study was to compare the agreement of a newly developed multipurpose probe to a standard linear carotid probe in detection of atherosclerosis of the precerebral arteries. ⋯ The multipurpose probe showed agreement with a standard linear probe in detecting atherosclerosis of the carotid arteries and has therefore the potential for use in both cardiac and precerebral ultrasound examinations.

Levodopa-responsive hemiparkinsonism secondary to cystic expansion from a coiled cerebral aneurysm.
An 80-year-old woman with longstanding hemifacial spasm had a 1 cm × 1.5 cm internal carotid artery terminus aneurysm treated with endovascularly delivered bare metal coils. Follow-up imaging revealed an expansile perianeurysmal cyst that coincided with development of contralateral dopa-responsive hemiparkinsonism. This is the first report of perianeurysmal cyst expansion causing levodopa-responsive hemiparkinsonism.

Mild cognitive impairment (MCI) precedes both Alzheimer's disease (AD) dementia and with Lewy bodies (DLB). We investigated proton magnetic resonance spectroscopy (MRS) characteristics of MCI patients who progressed to DLB compared to those who progressed to AD dementia or remained stable. ⋯ MRS may be useful in differentiating MCI patients with prodromal AD dementia from those with prodromal DLB for early disease-specific interventions.
