Case ReportsBrown papules and leukoderma in Darier's disease: clinical and histological features.
We report a Japanese case of Darier's disease with brown and white papules or maculae distributed on the neck, trunk and dorsa of hands. Both brown papules and leukoderma showed typical histological features of Darier's disease. Moreover, there were much fewer melanocytes and melanosomes in the epidermis of both lesions. ⋯ Therefore, the thick corneal layer of the brown papules may prolong the retention of a few melanosomes or a little melanin to induce hyperpigmentation, while the thin corneal layer of leukoderma may not do so, thereby producing hypopigmentation. The difference in clinical and histological courses after involvement of melanocytes was proposed to be the cause of the discrepancy between the previous reports. We conclude that leukoderma was the primary lesion, the postinflammatory depigmented spots, or the atypical or subclinical eruption of Darier's disease.

Case ReportsSubcutaneous emphysema and pneumomediastinum following dental extraction.
A case of acute subcutaneous emphysema of the lateral region of the neck is described. The patient, a 26-year-old woman, also had radiologic evidence of pneumomediastinum. She completely recovered in a few days under prophylactic antibiotherapy. In this case, subcutaneous and mediastinal emphysema was most probably due to the introduction of air into the soft tissue during dental surgery using compressed air equipment.

Case ReportsSystemic mastocytosis treated with histamine H1 and H2 receptor antagonists.
A case of systemic mastocytosis with skin characteristics of telangiectasia macularis eruptiva perstans is reported. Systemic involvement was confirmed as a combination of highly increased urinary excretion of methyl imidazole acetic acid (Melm AA) and increased amounts of mast cells in skin, liver and colon transversum. ⋯ The excretion of Melm AA was unchanged during treatment. After 7 months of continuous cimetidine and cyproheptadine treatment no side effects were observed.

Case ReportsTripe palms: a significant cutaneous sign of internal malignancy.
Tripe palms is a paraneoplastic keratotic skin sign of great predictive value. It is characterized clinically by a curious rugose thickening of the palms with an accentuation of the normal dermatoglyphic ridges and sulci. ⋯ We herein report a 66-year-old Chinese man with adenocarcinoma of the lung who presented typical tripe palms. Recognition of this distinctive pattern should prompt a meticulous search for an underlying malignancy, particularly lung or gastric carcinoma.
