Clinical TrialPropofol and remifentanil for deep sedation in children undergoing gastrointestinal endoscopy.
The aim of this study was to evaluate the safety and efficacy of a combination of propofol and remifentanil deep sedation in spontaneously breathing children less than 7 years of age undergoing upper and/or lower gastrointestinal endoscopy. ⋯ The combination of propofol and remifentanil for sedation in children undergoing gastrointestinal endoscopy can be considered safe, effective and acceptable.

Patient handover from surgery to intensive care: using Formula 1 pit-stop and aviation models to improve safety and quality.
We aimed to improve the quality and safety of handover of patients from surgery to intensive care using the analogy of a Formula 1 pit stop and expertise from aviation. ⋯ The introduction of the new handover protocol lead to improvements in all aspects of the handover. Expertise from other industries can be extrapolated to improve patient safety, and in particular, areas of medicine involving the handover of patients or information.

Case ReportsAirway management using the pediatric GlideScope in a child with Goldenhar syndrome and atypical plasma cholinesterase.
We report a case of difficult intubation in a child with Goldenhar syndrome and atypical plasma cholinesterase. Intubation attempts by direct laryngoscopy and the Trachlight were unsuccessful. The airway was ultimately secured using the pediatric GlideScope in conjunction with external laryngeal manipulation.

Our aim was to describe the incidence of quality assurance events between overweight/obese and normal weight children. ⋯ Studies on perioperative aspects of childhood overweight and obesity are rare. Our report shows a high prevalence of overweight and obesity in this cohort of pediatric surgical patients. Certain perioperative morbidities are more common in overweight and obese than in normal weight children. There is a need for prospective studies of the impact of childhood overweight and obesity on anesthesia and surgical outcome.

Postal survey of training in pediatric cardiac anesthesia in the United Kingdom.
There are no nationally agreed standards for training in pediatric cardiac anesthesia despite the recommendation of two reports. Since then, anesthesia training has changed because of the introduction of competency-based training, the New Deal and the European Working Time Directive. ⋯ They would recommend the same experience for others but this is unlikely to happen within the current constraints of specialist registrar training. Therefore, further training in a fellowship program in the UK or abroad is likely to be necessary.
