Clinical TrialUse of a remifentanil-propofol mixture for pediatric flexible fiberoptic bronchoscopy sedation.
Flexible fiberoptic bronchoscopy is an important diagnostic tool for pediatric pulmonologists. Because of its favorable respiratory profile, ketamine has become a popular sedative for this procedure, but may be associated with unpleasant emergence reactions in the older child. Remifentanil is a newer, ultra-short acting opioid that has been shown to provide effective sedation and cough suppression for fiberoptic bronchoscopy when combined with intermittent propofol boluses. However, delivery of these agents as a combined, single infusion has not been described. ⋯ A remifentanil/propofol mixture provided effective sedation and rapid recovery in pediatric patients undergoing fiberoptic bronchoscopy.

Continuous ventilation technique for laryngeal mask airway (LMA) removal after fiberoptic intubation in children.
Fiberoptic tracheal intubation through the laryngeal mask airway (LMA) is a simple technique to establish a safe airway in neonates and infants with a difficult airway. The technique, however, is complicated by the removal of the laryngeal mask from the patient's mouth because of the similarity in length of the LMA and the tracheal tube. ⋯ The technique is simple and safe, can be performed without hurry and carries potential advantages for neonates and children with limited cardiorespiratory reserve.

Randomized Controlled Trial Clinical TrialPostoperative behavioral changes following anesthesia with sevoflurane.
Behavioral disturbance following hospitalization is a relatively frequent event, some children still having negative behavioral changes (NBC) 1 month following their operation. Sevoflurane has a propensity to induce 'excitement' during induction of anaesthesia, and delirium in the immediate postoperative phase. The aim of this study was to evaluate whether this translates into prolonged behavioral change. ⋯ Children anesthetized with sevoflurane exhibit more immediate postoperative distress than those anesthetized with halothane. This difference is not carried over into the longer posthospital period. Negative behavioral changes occur more frequently with decreasing age.

Randomized Controlled Trial Comparative Study Clinical TrialTracheal sealing characteristics of pediatric cuffed tracheal tubes.
The aim of the study was to compare sealing characteristics of the new Microcuff pediatric tracheal tube featuring a high volume-low pressure (HVLP) cuff with ultrathin membrane with three conventional pediatric cuffed tracheal tubes. ⋯ This preliminary investigation suggests that the new Microcuff pediatric tracheal tube with ultrathin high volume-low pressure cuff membrane allows effective tracheal sealing at very low cuff pressures. This represents a benefit for children with regard to their lower mucosal perfusion pressures compared with adult patients.
