Case ReportsA pregnant woman with previous anaphylactic reaction to local anesthetics: a case report.
A 43-year-old woman with a history of lidocaine allergy required dermatologic surgery during pregnancy. To accomplish this without the use of general anesthesia, intradermal allergy testing was performed. This information enabled appropriate anesthetic care for a tissue biopsy at 29 weeks' gestation and subsequent labor analgesia and obstetric management at term.

Case ReportsArterial balloon occlusion of the hypogastric arteries: a life-saving procedure for severe obstetric hemorrhage.
In this case report we describe two patients with life-threatening postpartum hemorrhage who were treated successfully with arterial balloon occlusion of the hypogastric arteries. Arterial balloon occlusion is easy to perform and is applicable under difficult circumstances. The technique should be available in all hospitals that provide obstetric services. Obstetricians should know about it so they can ask their radiologic colleagues to perform the procedure.

Case ReportsHematometra of the rudimentary horn of a unicornuate uterus resulting from cesarean section.
A 30-year-old woman complained of increasing dysmenorrhea and progressive right lower-quadrant pain after cesarean section. A pelvic mass was observed and subsequently a congenital müllerian anomaly was diagnosed upon serial examination. We report the first apparent case of hematometra of the rudimentary horn of a unicornuate uterus resulting from cesarean section.

Comparative StudyIncreased risk of cesarean delivery with advancing maternal age: indications and associated factors in nulliparous women.
To investigate factors that contribute to the increased risk of cesarean delivery with advancing maternal age. ⋯ Older women are at higher risk for cesarean delivery in part because they are more likely to have cesarean delivery without labor. However, even among those women who labor, older women are more likely to undergo cesarean delivery, regardless of whether labor is spontaneous or induced. Part of the higher rate among older women who labor is explained by a higher rate of induction, particularly elective induction. Among women in both spontaneous and induced labor, cesarean delivery for the diagnoses of failure to progress and fetal distress was more frequent in older patients, although management of labor dystocia for these patients was similar to that for younger patients.

Randomized Controlled Trial Comparative Study Clinical TrialA comparison of various routes and dosages of misoprostol for cervical ripening and the induction of labor.
The purpose of this study was to compare the efficacy of different routes of misoprostol administration for cervical ripening and the induction of labor. ⋯ At the doses studied, induction of labor with vaginally administered misoprostol is more efficacious than either oral-plus-vaginal or oral-only route of administration.
