Does whole-spine lateral radiograph with clavicle positioning reflect the correct cervical sagittal alignment?
To evaluate the differences of cervical alignment between standing cervical lateral radiograph and whole-spine lateral radiograph with clavicle position. ⋯ Clavicle position during whole-spine radiograph caused a substantial decrease in the T1-slope; head position posteriorly translated followed by the cervical sagittal alignment became more hypo-lordotic, with slight downward gazing in comparison with the cervical radiograph.

They are selected on the basis that the author felt they were significant contributions to the spinal literature in that year. A brief precis is provided of each paper, and each is accompanied by a short discussion explaining its importance and relevance.

Impact on health related quality of life of adult spinal deformity (ASD) compared with other chronic conditions.
Medical and health policy providers should be aware of the impact of adult spinal deformity (ASD) on health-related quality of life (HRQL). The purpose of this study was to compare the relative burden of four chronic conditions with that of ASD. ⋯ The global burden of ASD was huge compared with other self-reported chronic conditions in the general population of eight industrialized countries. The impact of ASD on HRQL warrants the same research and health policy attention as other important chronic diseases.

Pedicle subtraction osteotomies (PSO) enable correction of spinal deformities but remain difficult and are associated with high complication rates. This study aimed to prospectively review different post-operative complications and mechanical problems in patients who underwent PSO as treatment for sagittal imbalance as sequelae of degenerative disc disease or previous spinal fusion. ⋯ Infection and neurologic complications following PSO are relatively common, and frequently reported in the literature. The principal cause of mechanical complications, such as non-union or junctional kyphosis, was insufficient sagittal correction, characterized by post-operative FBI >10°. The risks of insufficient correction are greater in patients with higher pelvic incidence and those patients who required very high correction.

To define reference parameters for analyzing sagittal balance of the cervical spine in asymptomatic volunteers. ⋯ The C7 slope has a predictive value of the shape of the cervical spine in the sagittal plane. One-third of the asymptomatic population had cervical kyphosis. Our results could be used to study sagittal balance before and after arthrodesis, or cervical prosthesis.
