We have demonstrated in a rat model that the intrathecal injection of 0.02 ml of 6.3% magnesium sulphate, a concentration iso-osmolar with rat plasma, will produce a state of spinal anaesthesia and general sedation, lasting approximately 1 h. These effects reversed completely after 6 h, without evidence of neurotoxicity, immediately or during the period 1 week following the injection. The accompanying changes in haemodynamic and respiratory functions were minimal throughout the period of anaesthesia and compare favourably with those induced by an intrathecal bolus of 0.04 ml of 2% lignocaine.

We report the case of a 30-year-old pregnant woman with an Arnold-Chiari malformation, who presented with severe pregnancy-induced hypertension at 30 weeks gestation. She was known to have had a difficult tracheal intubation during a previous general anaesthetic. Caesarean section was performed under an incremental epidural anaesthetic block.

A comparison of the effects of isoflurane and desflurane on the neuromuscular effects of mivacurium.
The effects of 1 MAC of desflurane and isoflurane (in 66% nitrous oxide) on the potency and duration of action of mivacurium were studied in 80 patients. The ED95 of mivacurium was 86 micrograms.kg-1 (74-100) and 88 micrograms.kg-1 (76-103) (mean and 95% confidence intervals) during anaesthesia with desflurane and isoflurane respectively. ⋯ There was no significant difference in any of the variables between desflurane and isoflurane. It is concluded that the neuromuscular effects of mivacurium are similar during anaesthesia with 1 MAC of desflurane and isoflurane.
