Early Use of Norepinephrine Improves Survival in Septic Shock: Earlier than Early.
The timing of initiation of Norepinephrine (NEP) in septic shock is controversial. ⋯ We concluded that early Norepinephrine in septic shock might cause earlier restoration of blood pressure, better lactate clearance and improve in-hospital survival.

Folic Acid and vitamin B12 supplementation improves coronary flow reserve in elderly subjects with vitamin B12 deficiency.
Major cardiovascular risk factors including hyperhomocysteinemia are frequently associated with decreased coronary flow reserve (CFR), an important physiological parameter of the coronary circulation. This study was designed to determine whether folate (5 mg) and vitamin B12 (500 μg) supplementation in elderly patients with vitamin B12 deficiency improved CFR, while reducing homocysteine levels. ⋯ In this study of elderly subjects with vitamin B12 deficiency, supplementation with folate and vitamin B12 was associated with a significant improvement in CFR values.

A comparison of three rating scales for measuring subjective phenomena in clinical research. II. Use of experimentally controlled visual stimuli.
In a previous study of three types of global scales we found that verbal rating scales were particularly reliable for rating auditory stimuli. We now wanted to check the performance of the scales for rating experimentally controlled visual stimuli. ⋯ Three instruments could be hierarchically ranked for their indices of validity and consistency. Being more consistent than VAS and NRS, VRS merits more frequent usage in clinical research.

Efficacy and safety of repeated postoperative administration of intramuscular diclofenac sodium in the treatment of post-cesarean section pain: a double-blind study.
Analgesic drugs, either opioids or non-opioids, are required and useful for controlling postoperative pain after cesarean section. ⋯ It might be concluded that repeated i.m. injections of 75 mg diclofenac sodium (maximum two injections per day) could relieve postoperative pain after cesarean section and significantly reduce opioid analgesic requirements without significant effects on uterine relaxation or bleeding during the first postoperative 48 h.

Rocuronium administration in children during isoflurane anesthesia: neuromuscular effects.
The time-course of the effect of rocuronium during isoflurane anesthesia in children has rarely been evaluated. Forty-five children, aged 2-14 years, ASA 1, undergoing elective surgery and receiving isoflurane anesthesia, were studied. ⋯ Maximal relaxation was reached in all children receiving 600 or 800 micrograms/kg of rocuronium. The TSRNMF were mainly related to the weight of the children, and gender affected each relationship. Widely variable responses were observed with all three doses.
