Randomized Controlled Trial Multicenter Study
A comprehensive care management program to prevent chronic obstructive pulmonary disease hospitalizations: a randomized, controlled trial.
Improving a patient's ability to self-monitor and manage changes in chronic obstructive pulmonary disease (COPD) symptoms may improve outcomes. ⋯ A CCMP in patients with severe COPD had not decreased COPD-related hospitalizations when the trial was stopped prematurely. The CCMP was associated with unanticipated excess mortality, results that differ markedly from similar previous trials. A data monitoring committee should be considered in the design of clinical trials involving behavioral interventions.

Idiopathic pulmonary fibrosis (IPF) is a progressive fibrotic lung disease with an overall poor prognosis. A simple-to-use staging system for IPF may improve prognostication, help guide management, and facilitate research. ⋯ The GAP models use commonly measured clinical and physiologic variables to predict mortality in patients with IPF.

Epidural steroids, etanercept, or saline in subacute sciatica: a multicenter, randomized trial.
Perineural inhibitors of tumor necrosis factor have recently generated intense interest as an alternative to epidural steroid injections for lumbosacral radiculopathy. ⋯ The John P. Murtha Neuroscience and Pain Institute, International Spinal Intervention Society, and Center for Rehabilitation Sciences Research.

Long-term efficacy of dapagliflozin in patients with type 2 diabetes mellitus receiving high doses of insulin: a randomized trial.
Dapagliflozin, a selective inhibitor of sodium-glucose cotransporter 2, may improve glycemic control with a lower dose of insulin and attenuate the associated weight gain in patients with inadequate control despite high doses of insulin. ⋯ AstraZeneca and Bristol-Myers Squibb.

Surrogate decision makers' interpretation of prognostic information: a mixed-methods study.
Little is known about why surrogate decision makers for patients with advanced illness often have overly optimistic expectations about prognosis. ⋯ National Heart, Lung, and Blood Institute.
