Case ReportsHerpes Simplex Virus Encephalitis: An Unexpected Outcome in a Polytrauma Patient.
An 84-year male was brought in the emergency after a road traffic accident leading to polytrauma with deteriorating consciousness. Prolonged unexplained unconsciousness led to cerebrospinal fluid examination. The polymerase chain reaction (PCR) of cerebrospinal fluid (CSF) was found to be positive for HSV-1 DNA. ⋯ The reasons to suspect herpes simplex virus encephalitis (HSE) in this case were unexplained worsening level of consciousness, CSF findings suggestive of viral encephalitis along with highly deranged alanine aminotransferase (ALT) levels. This case highlights the importance of keeping a high index of suspicion for viral encephalitis in patients with risk factors, even in such a scenario of polytrauma. Case ReportsRenal Clear Cell Carcinoma with Unknown Primary: Is It Real Entity or Just Hypothesis?
Renal cell carcinoma (RCC) is a common type of cancer, and about 25% of patients are diagnosed at an advanced stage. Bone metastasis is common in RCC, but instances of bone metastases without a primary kidney tumour have rarely been reported. Here, we report a case of a 56-year male patient who presented with extensive bone metastases. ⋯ In the literature, primary unrecognised RCC cases are found only as case reports. This case highlights the importance of clinicians' consideration of possible metastatic RCC in a patient with pathologically diagnosed clear cell carcinoma, even if no primary renal tumour is identified. Case ReportsOsteoporotic Vertebral Fracture Misdiagnosed as Metastatic Vertebral Fracture.
The purpose of this study was to report a patient with osteoporotic vertebral compression fracture (OVCF) which was misdiagnosed as metastatic vertebral compression fracture (MVCF). A 64-year male was admitted to our clinic for complaints of numbness, pain, and activity limitation in bilateral lower extremities. One year ago, he had a medical history of lung adenocarcinoma and bone metastasis. ⋯ It is important to differentiate OVCF from MVCF by clinical symptoms, laboratory examination, and imaging features before operation. Histological findings are the gold standard for accurate diagnosis. Key Words: Osteoporosis, Vertebral fracture, Metastasis.
