Meta AnalysisImpact of Resistance Training on FEV1 and Functional Exercise Capacity among COPD Patients: A Meta-analysis.
Chronic obstructive pulmonary disease (COPD) is the third leading cause of mortality and morbidity across the Globe. A meta-analysis was conducted to determine the effectiveness of different modes of resistance training on FEV1 and functional exercise capacity among COPD patients. Data bases of Cochrane, PEDro, Embase, CINAHL, PubMed and Google Scholar were searched seeking research articles of interest. ⋯ Results of the study demonstrated that resistance training has a mild pool effect in increasing FEV1 with an effect size of (SMD) of 0.160 (95% CI of -0.840 to 0.521) calculated at random effect model, I2 = 62.99% (95% CI of 0.00 to 89.42); whereas, functional exercise capacity demonstrated large pool effect size 0.886 (95% CI of 0.401 to 1.371) with I2 = 0.0 (95% CI of 0.00 to 0.00). The study concludes that there is a small to large impact of resistance exercises in improving functional exercise capacity and forced expiratory volume. Hormonal Changes in Clomiphene Citrate Cycles in Unexplained Infertility Can Predict Follicle Development?
To examine the intra-cycle and inter-cycle hormonal changes in the clomiphene citrate (CC) cycle in women with unexplained infertility; and to determine the factors that may predict follicle development or CC failure. ⋯ Basal serum FSH, P4 and DHEA‑S levels can be used as predictors of ovulation in CC cycles in women with unexplained infertility by determining a threshold value with more comprehensive studies to be conducted in the future. Case ReportsManagement of Gastric Tumor Perforation with Conservative Treatment during Neoadjuvant Chemotherapy.
Perioperative chemotherapy provides advantage for gastric cancer patients in terms of survival. A 56-year male with a diagnosis of locally advanced gastric carcinoma presented with complaints of acute abdominal pain; and was diagnosed as gastric tumor perforation during neoadjuvant therapy. Gastric perforation may occur during neoadjuvant chemotherapy for gastric cancer. ⋯ We used the minimally invasive treatment approach with nasogastric drainage, intravenous antibiotics and proton pump inhibitors, which was effective in the treatment of perforation. Althrough, conservative treatment approach may be an effective management option in selected patients with gastric cancer perforation. Case ReportsSuccessful Pregnancy Outcome in a Lady with Polyglandular Autoimmune Syndrome Type II.
We report a case of a lady who had polyglandular autoimmune syndrome type II (hypothyroidism, Addison's disease, vitiligo), who completed a successful pregnancy. Addison's disease (AD) was confirmed by the presence of anti-adrenal antibodies and hyponatremia. The patient had pre-pregnancy counselling about the need of antepartum, intrapartum and postpartum steroids. ⋯ The fetal growth was at 90th centile. Management in pregnancy, labour and postnatal period requires multidisciplinary care by the endocrinologist, obstetrician, midwife, anaesthetist and neonatologist. Case ReportsDouble Balloon Enteroscopy in a Pregnant Woman with Peutz-Jeghers Syndrome.
Peutz-Jeghers syndrome is an autosomal dominant disorder characterised by multiple hamartomatous polyps and muco-cutaneous pigmentation. Most polyps are found in the small intestine, especially in the jejunum. Enteroscopy with polypectomy is a reliable method to prevent polyp-related complications. ⋯ This procedure was the first double balloon enteroscopy during pregnancy, reported in the literature. The procedure should be performed by experienced endoscopists, who can cope well with the complications in expert centres, as the procedure carries a high risk.
