Correlation between the educational environment and burn-out syndrome in residency programs at a university hospital.
The educational environment (EE) is related to satisfaction and achievement. A high prevalence of professional burnout (BO) syndrome has been reported in the residency. The objective of this study was to evaluate the correlation between EE and the presence of burnout in residency programs at a university hospital. ⋯ In the residency, a significant correlation was found between the EE and burnout (reverse with exhaustion and depersonalization, and direct with personal accomplishment). This correlation had greater strength and the same sense for items evaluating autonomy.

Observational Study[Onychomadesis associated with mouth, hand and foot disease].
Onychomadesis is the spontaneous, complete shedding of the nail from its proximal side, without pain or inflammation, following nail matrix arrest. This disorder is uncommon in children and it can occur in fingernails, toenails or both. It may be secondary to systemic disorders, Kawasaki disease, bullous dermatoses, drugs, paronychia, stress and radiotherapy. ⋯ HFMD is an infection characterized by vesicular and erosive stomatitis in combination with a vesicular eruption in palms and soles. It occurs in small children during summer and autumn months, and it is caused by coxsackie virus. We present a study that reflects the current situation of onychomadesis in Argentinian children and shows a strong association between this disorder and HFMD, suggesting that onychomadesis is a new manifestation of a previously known disease.

Observational StudyAcute kidney injury in critically ill children: incidence and risk factors for mortality.
Acute kidney injury is a common complication associated with an increase in mortality in children who require intensive care. The objective of this study was to determine the incidence of acute kidney injury and identify risk factors for mortality in critically ill patients hospitalized in our facility. ⋯ The incidence of acute kidney injury in critically ill children was 4.4%, and the need of dialysis was an independent predictor of mortality.

Review[Invasive mechanical ventilation: Update for the pediatrician].
In this review, we collect the fundamental concepts of the use of invasive mechanical ventilation (MV) in children, particularly in acute respiratory failure. MV is a common practice in the ICU and must be understood as a therapeutic intervention to replace the work of breathing while restores the balance between ventilatory demand and the patient's ability to sustain it. It is essential for the clinician to recognize that the goal of mechanical ventilatory support is not to normalize the patient's blood gases but providing a reasonable gas exchange; the benefts are obtained if the safety thresholds are not exceeded. Thus, this strategy has become the only tool available to limit the development of ventilator-induced lung injury (VILI).

Impact of a quality improvement intervention on the incidence of unplanned extubations in a Pediatric Intensive Care Unit.
An unplanned extubation (UEX) is a frequent and preventable adverse event of mechanical ventilation. ⋯ The most common causes of UEX were the incorrect fxation of the endotracheal tube and the use of a procedure of care. The implementation of a quality improvement intervention reduced the incidence of UEX.
