Randomized Controlled Trial Multicenter StudyRationale and design of the Randomized Intervention with CPAP in Coronary Artery Disease and Sleep Apnoea--RICCADSA trial.
Obstructive sleep apnoea (OSA) is common in coronary artery disease (CAD) and a possible cause of increased mortality. To date, there is a lack of randomized controlled trials to draw the conclusion that all CAD patients should be investigated for OSA and subsequently be treated with continuous positive airway pressure (CPAP). ⋯ The RICCADSA trial will contribute to defining the impact of CPAP on prognosis of CAD patients with OSA.

Randomized Controlled TrialAssociation of edge-to-edge repair to De Vega annuloplasty for tricuspid incompetence.
We aimed to analyze whether association of edge to edge repair to De Vega annuloplasty would result in better results in patients with severe TR. ⋯ Our results suggested that association edge-to-edge technique to De Vega annuloplasty has resulted in reduced residual TR and less incidence of recurrence of TR and TVA dilatation.

Randomized Controlled TrialInsulin resistance after cardiopulmonary bypass in the elderly patient.
Preoperative carbohydrate administration attenuates insulin resistance. We studied effects of preoperative oral carbohydrate loading in elderly patients undergoing coronary artery bypass grafting. ⋯ The study group was insulin resistant on postoperative day one and two. The effects were explainable by the traumatic stress response. No adverse effect was noted from the carbohydrate drink. If glucose is administered intravenously during surgery, there is no obvious advantage of preoperative carbohydrate loading on insulin resistance or stress hormone response.

Randomized Controlled TrialDoes preemptive stellate ganglion blockage increase the patency of radiocephalic arteriovenous fistula?
Radio-cephalic arteriovenous fistulas (AVFs) have high early failure ratio. Increased sympathetic activity and spasm of radial artery during the surgery may responsible for early occlusion rate. ⋯ AVF occlusion rate is much more common in early postoperative period. Diminished sympathetic tonus by preemptive SGB not only increases early patency rate but also increases fistula maturation rate.

Randomized Controlled TrialPericardial suction blood separation attenuates inflammatory response and hemolysis after cardiopulmonary bypass.
Retransfusion of pericardial suction blood (PSB) is critically considered under the aspect of the biocompatibility of the cardiopulmonary bypass (CPB). We investigated various indicators of inflammation and blood cell activation associated with CPB and re-transfusion of PSB during cardiac surgery. ⋯ Cardiotomy suction is a major cause of hemolysis and contributes significantly to the systemic inflammatory response.
