The significance of acute kidney injury in Clostridioides difficile infection.
Clostridioides difficile infection (CDI) may present as sepsis with acute kidney injury (AKI). Herein, we aimed to evaluate the clinical outcomes of patients with AKI complicating CDI. ⋯ CDI patients with AKI have an increased risk of mortality and further deterioration of renal function. Early renal function recovery does not infer protection from further deterioration of renal function at 3 months. Caution and nephrology follow-up should be considered after discharge for all patients who developed AKI during CDI, regardless of discharge creatinine levels.

Ferritin index is a strong prognostic marker in Adult Hemophagocytic Lymphohistiocytosis.
The relationship between ferritin levels and survival in adult hemophagocytic lymphohistiocytosis (HLH) has been evaluated in previous studies. However, Admission-to-discharge percentage ferritin reduction (named as ferritin index) level in adult patients with HLH has never been evaluated to predict 6-month survival. ⋯ Ferritin index is a better predictor of 6-month survival than admission and discharges ferritin levels in adult patients with HLH.

Thoracic endosonography (EBUS/EUS-b) in diagnosis of different intrathoracic diseases: a 4-year experience at a single-center in Greece.
In the last decade, the advent of thoracic endosonography has revolutionised the field of diagnostic bronchoscopy. ⋯ Thoracic endosonography is an efficient, safe, minimally invasive tool yielding high sensitivity and diagnostic accuracy in patients with suspected malignancy and mediastinal lymphadenopathy. Experienced pulmonologists in EBUS-TBNA should more routinely perform EUS-b FNA to avoid unnecessary surgical interventions.

Treatment Intensification in Type 2 Diabetes Management after the Failure of Two Oral Hypoglycemic Agents: A Non- Interventional Comparative Study.
The safety and efficacy of treatment approaches in patients with type 2 diabetes mellitus (T2DM) after the failure of two oral hypoglycemic agents (OHAs) was studied. ⋯ Insulin as add-on therapy with OHAs is an option for the management of T2DM where glycemic control is insufficient with two OHAs.
