Meprobamate tablets contain microcrystalline cellulose, a potent embolic agent that has been shown to cause gangrene in animal studies. Microvascular embolization caused by microcrystalline cellulose can contribute to the ischemic process. ⋯ We believe this is the first case of acute limb ischemia caused by intra-arterial injection of meprobamate powder documented in humans. Emergency physicians should be aware that accidental intra-arterial injection of crushed oral drug formulations is potentially limb threatening and prompt recognition of similar clinical scenarios is of vital importance.

Hyperglycemia-induced hemiballismus hemichorea: a case report and brief review of the literature.
Metabolic conditions, including hyperglycemia, can have various neurological presentations. Hemiballismus hemichorea is a rare manifestation reported to occur with severe hyperglycemia and is reversed in most cases with control of sugars. ⋯ Important differential diagnoses that need to be evaluated are discussed.

The presence of free intraperitoneal gas usually warrants emergent surgery. In rare instances, however, non-surgical conditions such as jejunal diverticulosis can cause pneumoperitoneum and do not require intervention. ⋯ Although spontaneous pneumoperitoneum due to jejunal diverticulosis is a rare finding, when it does occur, this condition must be distinguished from other forms of pneumoperitoneum to avoid unnecessary surgery. CT scan findings of multiple rounded, variably sized jejunal outpouchings filled with oral contrast are helpful in diagnosing jejunal diverticulosis and confirming the decision for conservative management of the patient.
