Transoral laser microsurgery for advanced laryngeal cancer.
To report the oncologic and functional outcomes of transoral laser microsurgery (TLM) in the treatment of advanced laryngeal cancer. ⋯ In patients with advanced laryngeal cancer, TLM with or without radiotherapy is a valid treatment strategy for organ preservation. Furthermore, low morbidity and mortality and excellent oncologic and functional outcomes make TLM an attractive therapeutic option.

Construct validity of the endoscopic sinus surgery simulator: II. Assessment of discriminant validity and expert benchmarking.
To establish discriminant validity of the endoscopic sinus surgery simulator (ES3) (Lockheed Martin, Akron, Ohio) between various health care provider experience levels and to define benchmarking criteria for skills assessment. ⋯ This study completes the construct validity assessment of the ES3 by demonstrating its discriminant capabilities. It establishes expert surgeon benchmark performance criteria and shows that the ES3 can train novice subjects to attain those. The refined analysis of trial performance scores could serve educational and skills assessment purposes. Current studies are evaluating the transfer of surgical skills acquired on the ES3 to the operating room (predictive validity).

Radiographic classification of temporal bone fractures: clinical predictability using a new system.
To compare the traditional system of radiographic classification of temporal bone fractures (transverse vs longitudinal vs oblique) with a newer system (otic capsule violating vs otic capsule sparing) with respect to their ability to predict sequelae of temporal bone trauma. ⋯ The traditional radiographic classification system failed to demonstrate clinical predictability in our series. Furthermore, the newer system of classification (otic capsule sparing vs otic capsule violating) demonstrated statistically significant predictive ability for serious clinical outcomes associated with temporal bone fractures.

Prospective study of perioperative factors predicting hypocalcemia after thyroid and parathyroid surgery.
To identify whether perioperative 1,25-dihydroxyvitamin D or parathyroid hormone (PTH) levels will predict the development of hypocalcemia after thyroid and parathyroid surgery. ⋯ Preoperative 1,25-dihydroxyvitamin D levels were not predictive of postoperative calcium levels. Patients who undergo PAE or TL are at extremely low risk for requiring calcium replacement. Patients who undergo TT or SP with 8-hour postoperative PTH levels greater than or equal to 15 pg/mL (1.6 pmol/L) are at low risk for developing postoperative hypocalcemia, whereas those with PTH levels less than 15 pg/mL (1.6 pmol/L) have a high risk of developing hypocalcemia.

A novel organ preservation protocol for advanced carcinoma of the larynx and pharynx.
To pilot a targeted chemoradiation protocol for patients with advanced carcinoma of the larynx and pharynx that would circumvent upper aerodigestive tract dysfunction related to major oncologic surgery. ⋯ We believe this chemoradiation protocol represents an effective management scheme for patients with advanced head and neck cancer while minimizing dysfunction and possibly improving survival.
