Randomized Controlled Trial Comparative StudyComparison of the efficacy of femoral nerve block and fascia iliaca compartment block in patients with total knee replacement.
Blocking the femoral nerve reduces postoperative pain and analgesic consumption in patients who have undergone total knee and hip replacement surgery. A limited number of studies have compared the efficacy of the fascia iliaca compartment and femoral nerve block techniques. Therefore, this study aimed to investigate the analgesic effectiveness of fascia iliaca compartment block (FIB) and femoral nerve block using ultrasound. ⋯ Femoral nerve block provided more potent analgesia in the first six hours after operation. After six hours, FIB demonstrated better pain control. The quality of postoperative recovery was higher in patients with fascia iliaca compartment block.

Myocardial injury after non-cardiac surgery: a perioperative affair?
Myocardial injury after non-cardiac surgery (MINS) is a rather new nosological entity and an unfortunately common perioperative complication. The diagnostic criteria for MINS, also indicated as isolated myocardial injury (IMI), are an elevated postoperative high sensitivity troponin T (hsTnT level ranging between 20 and 65 ng/L with an absolute change of at least 5 ng/L or hsTnT level >65 ng/L), in absence of symptoms and/or EKG findings suggestive of ischemia and without a non-ischemic etiology causing troponin elevation. MINS does not fulfill the universal definition of myocardial infarction even if it is related to ischemic causes and it is independently associated with 30-day postoperative mortality and complications. ⋯ Physiopathology of MINS is not yet well defined: current hypotheses are surrogated from perioperative myocardial infarction studies. Up to now there are not specific treatments for MINS, even if antithrombotic therapy is under evaluation. Treatment decisions should be tailored to the individual case; potential benefits of troponin screening include a cardiology consultation and consequently, improved patients' information to promote lifestyle changes and enhanced therapy.

Observational StudyInflammation and primary graft dysfunction after lung transplantation: CT-PET findings.
The leading cause of early mortality after lung transplantation is Primary graft dysfunction (PGD). We assessed the lung inflammation, inflation status and inhomogeneities after lung transplantation. Our purpose was to investigate the possible differences between patients who did or did not develop PGD. ⋯ Compared to healthy lungs, all the grafts showed increased [18F]FDG uptake rate, but there were no differences between patients who developed PGD and patients who did not. Of note, the PGD patients showed a worse inflation status of lungs and a higher inhomogeneity extent.
