In this paper we discuss the Neurosurgical Training Programme (NTP) in some European countries. ⋯ The purpose of this paper is to compare and contrast several NTP's in Europe in order to promote a more coherent medical education. Some remarks and suggestions from the perspective of young neurosurgeons are given.

Surgical approaches to Meckel's cave (MC) are often technically difficult and sometimes associated with postoperative morbidity. The relationship of surgical landmarks to relevant anatomy is important. Therefore, we attempted to delineate quantitatively their anatomy and the relationships between MC and surrounding structures. ⋯ The anatomical landmarks as presented herein regarding MC may be used for a safer skull base approach to the region.

Pneumorrhachis associated with bronchial asthma, subcutaneous emphysema and pneumomediastinum.
Pneumorrhachis is defined as the presence of air in the epidural space or subarachnoid space. The air may migrate along fascial planes from the posterior mediastinum, through the neural foramina, and into the epidural space. ⋯ The combination of pneumomediastinum with epidural pneumorrhachis without thoracic trauma has rarely been reported in the literature. The present case report describes the presence of pneumomediastinum, subcutaneous emphysema, and pneumorrhachis in a child asthmatic patient who had a history of fever, violent cough.

Correlation of pulsatility index with intracranial pressure in traumatic brain injury.
In traumatic brain injury (TBI) patients, it is desired to monitor the intracranial pressure (ICP) to assess the cerebral haemodynamics and guide the therapy. The study was designed to see if the pulsatility index (PI) measured by transcranial Doppler ultrasonography (TCD) predicts information about ICP values. ⋯ The strong correlation observed between ICP and PI through the management period of TBI patients can lead us to use TCD ultrasonography-derived PI as a guide if invasive monitoring is not available.

Recurrent cerebral arteriovenous malformation with a posterior inferior cerebellar artery aneurysm.
Cerebral arteriovenous malformations (AVMs) are congenital lesions which rarely recur after complete microsurgical excision. ⋯ Although some authors do not propose routine additional follow-up in adults after the initial negative postoperative angiogram, we believe that such patients should be followed clinically and radiologically.
