The medical infrastructure of Iraqi Kurdistan, a semiautonomous region in the northern part of Iraq, lags disproportionately behind relative to the otherwise booming industrial advances of the region. Although neurosurgical training is available, the local population lacks trust in its own neurosurgeons. ⋯ Over the last 4 years, the visiting neurosurgical team has seen hundreds of patients in consultation and performed more than 50 complex cranial and spinal operations jointly with local neurosurgeons. This article discusses our experience as volunteer neurosurgeons in building neurosurgical capacity in Iraqi Kurdistan.

Intradural Extramedullary Capillary Hemangioma in the Upper Cervical Spine: First Report.
The occurrence of intradural extramedullary capillary hemangiomas is exceedingly rare. To date, only 39 cases of intradural extramedullary capillary hemangiomas have been reported in the English literature, and all of these cases have been described at the lumbar and thoracic spinal levels. To our knowledge, this report is the first case of capillary hemangiomas of the cervical spine in the literature. In general, this entity is misdiagnosed preoperatively as a neoplasm. ⋯ Although rare, our case demonstrates that capillary hemangioma should be considered in the differential diagnosis of intradural extramedullary tumor of the cervical spine.

Headache is a controversial indication for treatment of pituitary adenoma. We studied the possible relationship between pituitary adenomas and headache as well as the symptomatic effects of treatment. ⋯ In a cohort with both treated and untreated patients with pituitary adenoma, headache prevalence was low compared with the general population. We found no link between clinical or radiologic variables and headache. Although a higher proportion of patients who underwent surgical treatment reported symptomatic relief, most patients who did not undergo treatment also improved. We believe that the unpredictable dynamics of headache over time and the lack of predictive and modifiable tumor-related variables associated with headache or treatment of headache weaken headache as a sole indication for pituitary adenoma treatment.

Pseudoaneurysm at the distal posterior inferior cerebellar artery after blunt head trauma: A case report and literature review.
Traumatic pseudoaneurysm of the distal posterior inferior cerebellar artery (PICA) is extremely rare. We report our experience of a case of pseudoaneurysm that developed on the PICA after blunt trauma to the head. ⋯ Given that development of traumatic pseudoaneurysm usually is delayed, follow-up radiologic examination is required, especially for patients with severe blunt trauma.
