Epidural steroid injection for low back and leg pain has been shown to result in a positive clinical outcome. Lumbosacral selective nerve root block (SNRB) via a transforaminal approach is commonly performed under fluoroscopic guidance. However, ultrasound guidance is an alternative to overcome the radiologic disadvantages. This study examined the accuracy of needle-tip localization under ultrasound guidance with a contrast bubble, compared with fluoroscopic guidance. ⋯ In order to avoid radiation exposure during the SNRB procedure, ultrasound guidance might be an alternative. Despite being accepted practice, there is a steep learning curve involved in the use of ultrasound guidance for lumbar SNRB, and proper training is crucial. A contrast bubble is a prospective enhancement for better visualization of ultrasound guidance.

Distinctive Surgical Strategy for Removal of String of Beadlike Schwannomas of Cauda Equina.
Spinal schwannomas are the most common intradural extramedullary tumors. However, a string of beadlike schwannomas is rare. In some cases, the beadlike tumors might present a multiple segmental growing pattern, often located in the lumbar spinal canal and on 1 nerve fiber. Despite its benign nature, the resection of this string of beadlike tumors could be a challenge to neurosurgeons, especially when the tumors extend to a long distance. ⋯ In this case, we first reported a new surgical approach for the removal of a string of beadlike cauda equina schwannomas. We hypothesize that this procedure would be a potential addition to the present surgical methods under some circumstances.

The surgical Simpson grade, introduced in 1957, is the standard measure for meningioma resection and prediction of recurrences. We used an magnetic resonance (MR)-based grading system for the radiologic extent of resection, and assessed agreement of the extent of resection between the surgical Simpson grade and the MR-based scale. ⋯ Agreement for extent of meningioma resection between both scales was good in terms of the ICC. When the surgical Simpson grade is unclear, MR imaging at 3 months after surgery may be used as a baseline for further follow-up. In a substantial portion of cases, the extent of resection was less favorable on the early postoperative MR imaging than the surgeon's Simpson grade. The predictive value of the radiologic extent of resection for the risk of long-term recurrences is a subject for further research.

The impact of obesity on spine surgery has been studied extensively, but only a few investigations have been focused on minimally invasive spinal fusion techniques and complication rates in normal-weight, preobese, or obese patients. Obesity was found to be a risk factor for intraoperative complications. Published data tend to favor minimal access surgery techniques (MAST) for obese patients. In a prospective study, we assessed the perioperative and postoperative complications of MAST in a large population of 187 patients. ⋯ We conclude that preobese and obese patients are good candidates for MAST because BMI did not affect complication rates or duration of surgery.

To investigate the prognostic factors for microvascular decompression (MVD) in patients with primary trigeminal neuralgia (TN), with a particular focus on the morphology of the posterior cranial fossa (PCF). ⋯ The present study identified type 2 TN, weak neurovascular compression, and flatness of the PCF as predictors of poor prognosis after MVD for primary TN.
