Completion corpus callosotomy with stereotactic radiosurgery for drug-resistant, intractable epilepsy: Case report.
Stereotactic radiosurgery (SRS) offers a noninvasive technique for division of the corpus callosum, which can confer improved seizure control to patients suffering from frequent atonic seizures due to rapid interhemispheric generalization. This noninvasive approach is well-suited for use in a palliative intervention for improved seizure control in this patient population. To our knowledge, this is the first report of radiosurgical completion corpus callosotomy in an adult in the United States. ⋯ Eight months postradiosurgical corpus callosotomy, the patient is free of atonic seizures and is ambulatory. In carefully selected cases and with protective radiosurgical planning, SRS for completion corpus callosotomy represents an effective option for refractory seizure control.

Iatrogenic cerebrospinal fluid (CSF) leaks are associated with serious sequelae. We analyzed the causes, complications, treatments, and outcomes of iatrogenic CSF leaks. Furthermore, the comorbidities and outcomes between timely and delayed repairs of iatrogenic CSF leaks were also compared. ⋯ Delayed intervention of CSF leak after transsphenoidal surgery and craniotomy was observed, increasing the risks of meningitis. Timely intervention with grafts based on the defect sites and patient prior surgical history is crucial for avoiding life-threatening comorbidities.

We describe the dural relationships and its surgical implications for large lower cranial nerve (CN) neurinomas. The study is based on surgical experience with 14 cases. ⋯ The "dural" cover of the lower CN neurinomas forms a strong and reliable plane of compartmentalization and allows safe tumor resection.

Although rotation is an important aspect of pathogenesis of adolescent idiopathic scoliosis (AIS), there are no studies demonstrating rotation pattern by directly measuring rotation angle in computed tomography (CT) images. The present retrospective comparative radiographic study was conducted to evaluate the rotation pattern in AIS and its relation to curve morphology and Lenke classification. ⋯ Curves with different Lenke types demonstrated different levels of MRV and severity of rotation. The results suggest that different center levels of rotation, signified by MRV, are a factor determining curve morphology. The findings would be a basis of connection between 2-dimensional classification and transverse plane deformity.

Tuberculum Sellae Meningiomas inPregnancy: 3 Cases Treated in the 2nd Trimester & Literature Review.
Tuberculum sella meningiomas typically present with progressive visual loss. It is also known that meningiomas can become symptomatic during pregnancy. Herein we report on 3 patients who presented with progressive visual decline during pregnancy, prompting urgent surgical removal of their meningiomas. ⋯ Presentation of tuberculum sella meningioma during pregnancy is uncommon but not rare, accounting for 7% of women in our series. Ideally, surgery is performed in the second trimester, ensuring fetal safety while restoring maternal vision and maintaining pituitary gland function are essential. Depending on tumor size and sellar anatomy, endoscopic endonasal or supraorbital keyhole craniotomy approaches are both viable options.
