The use of cortical bone trajectory (CBT) pedicle screws for posterior fixation and fusion seems to constitute a viable alternative for spinal procedures, with the potential to mitigate risks, be minimally invasive, and cause less tissue damage than the traditional technique. This review analyzes the literature regarding CBT according to the rate of evidence of articles and their main focus. ⋯ Through the years, surgical technique has improved to gain a longer and safer trajectory than first described. Level 2 and 3 clinical studies suggest equal clinical and radiologic outcomes compared with pedicle trajectory fixation, but high-quality, level 1, randomized controlled trials are needed to confirm these results.

Prospective study on the efficacy of orally administered tranexamic acid and Goreisan for the prevention of recurrence after chronic subdural hematoma burr-hole surgery.
This prospective study investigated whether tranexamic acid and Goreisan effectively prevent recurrence after burr hole surgery for chronic subdural hematoma. ⋯ Oral administration of tranexamic acid or Goreisan does not minimize recurrence after chronic subdural hematoma burr hole surgery; however, tranexamic acid can reduce the hematoma volume.

Traditional retraction to access deep intraparenchymal brain lesions results in vascular disruption. Tubular retractors such as the BrainPath tubular retractor system were developed to reduce retractor-related force injuries via radial dispersion of force. Our study seeks to assess the indications, benefits, and complications associated with BrainPath retractors. ⋯ This report is the first formal assessment of the BrainPath tubular retraction system, highlighting technical considerations of the retractor from the surgeon's perspective, patient outcomes, and complications. The retractor is a safe, efficacious system that can be used for tumor resection or biopsy and hematoma evacuation. However, further randomized controlled trials are indicated to accurately assess complication rates and outcomes.

Artificial Discs in Cervical Disc Replacement: A Meta-Analysis for Comparison of Long-term Outcomes.
Cervical disc replacement (CDR) has emerged as an alternative to anterior cervical discectomy and fusion for the surgical treatment of degenerative cervical disc disease. Although comparison of the 2 techniques has been studied in the literature, a thorough assessment of all artificial discs between each has not been performed. The objective of the present study was to examine the long-term outcomes of 5 artificial discs. ⋯ The results of the present meta-analysis indicate that surgical and clinical outcomes may differ among different CDR devices. These findings may assist surgeons in tailoring their decision making to specific patient profiles. Future multicenter efforts are needed to validate associations found in this study.

Magnetic resonance-guided laser interstitial thermal therapy is a minimally invasive procedure that uses intraoperative magnetic resonance thermometry (MRT) to generate a thermal damage estimate (TDE) of the ablative area. This study aimed to compare areas produced by the MRT heat map with the system-generated TDE produced by Visualase software. ⋯ The system-generated TDE consistently remains contained within the boundaries of the MRT heat map. At standard factory settings, TDE and the area produced within the periphery of HM63.9 are similar in magnitude. The light blue portion of the MRT heat map may serve as an additional means of predicting when critical structures may be at risk during laser ablation if exposed to further thermal stress.
