Clinoidal meningiomas are meningiomas arising from or in the vicinity of the anterior clinoid process.1 Despite advanced microsurgical techniques, clinoidal meningiomas remain challenging.2 Extradural anterior clinoidectomy with optical unroofing remains an important tool in skull base surgery, which provides a safe operative corridor, facilitating greater extent of resection and enhancing overall outcome, particularly visual function.2-13 A 66-year-old woman presented with history of visual disturbances. Magnetic resonance imaging revealed a dural-based tumor consistent with a large left clinoidal meningioma, with tumor wrapping (encircling) around the left trunk and internal carotid artery (ICA) bifurcation, elevating the left middle cerebral artery M1 segment, and invading the left optic canal. Left cranio-orbital craniotomy with pretemporal exposure was used (Video 1).1,9 A high-speed diamond drill with irrigation completed the extradural anterior clinoidectomy and optical canal unroofing. ⋯ I. A.), essentially "unwrapping" the left ICA trunk and its bifurcation. The patient consented to publication.

Verifying the intervertebral stability of each intervertebral fusion procedure, including transforaminal, posterior, and lateral lumbar interbody fusion (TLIF, PLIF, and LLIF, respectively), and the ratio of stress on the rods and pedicle screws during initial fixation may help select a fixation procedure that reduces the risk of mechanical complications, including rod fracture and screw loosening. Thus, we aimed to assess whether these procedures could prevent mechanical complications. ⋯ Stress on the rods and pedicle screws in the LLIF model was the lowest compared with that induced by other intervertebral fusion procedures. Therefore, LLIF may reduce mechanical complications occurrence, including rod fracture and screw loosening.

In cerebral aneurysm coil embolization, proper microcatheter shaping is crucial to reduce complications and achieve sufficient embolization. Shaping a microcatheter in 3 dimensions (3D) is often required but can be challenging. We assessed the usefulness of a novel shaping on screen (SOS) method that displays real-size 3D rotational angiography (RA) images on a touch screen device during cerebral aneurysm embolization to facilitate 3D microcatheter shaping. ⋯ Three-dimensional (3D) microcatheter shaping using touch screen devices during cerebral aneurysm coil embolization may be simple and safe and can achieve high packing density of aneurysms.

We aim to elucidate the contribution of early dynamic changes in the neutrophil-to-lymphocyte ratio (NLR) to poor clinical outcomes in acute ischemic stroke patients after endovascular treatment (EVT). ⋯ The NLR in acute ischemic stroke patients increased remarkably independent of successful reperfusion. Elevated changes in the NLR might predict malignant hemorrhagic transformation, adverse functional outcomes, and short-term mortality.

The goal of this study is to implement machine learning (ML) algorithms to predict mortality, non-home discharge, prolonged length of stay (LOS), prolonged length of intensive care unit stay (ICU-LOS), and major complications in patients diagnosed with thoracolumbar spinal cord injury, while creating a publicly accessible online tool. ⋯ ML models demonstrate good predictive ability for in-hospital mortality and non-home discharge, fair predictive ability for major complications and prolonged ICU-LOS, but poor predictive ability for prolonged LOS. We have developed a web application that allows these models to be accessed.
