Meningiomas are the most common primary central nervous system tumors, often requiring surgical resection. Presurgical embolization (PSE) is used to reduce intraoperative bleeding, although its effectiveness varies. This study evaluates the safety and efficacy of PSE using ethyl-vinyl alcohol (EVOH) in meningioma surgeries. ⋯ PSE using EVOH significantly decreases surgical bleeding and time in meningioma resections without associated complications. The proposed PSE grading system (Preoperative Embolization of Meningiomas, Mexican Classification) may enhance the predictability of intraoperative bleeding control. While we acknowledge the relatively small sample size of 48 cases, we believe this study serves as a foundation to encourage the inclusion of more patients and collaboration with additional centers, aiming to further validate and standardize the classification.

This study updates the results of a facial nerve preservation approach to treating vestibular schwannomas at a single institution by a single surgeon using retrosigmoid craniectomy with microsurgery (MS) and gamma knife stereotactic radiosurgery (SRS) over a 25-year interval. ⋯ For larger tumors, MS emphasizing facial nerve preservation over gross total resection followed by adjuvant SRS for any interval tumor regrowth yields excellent facial nerve outcomes and tumor control rates. For smaller tumors, upfront SRS achieves superior facial nerve preservation with MS and SRS both viable salvage options for treatment of tumor progression. This paradigm balances tumor control, facial nerve preservation, and low morbidity.

The study aims to evaluate the outcomes of surgical intervention in eclamptic patients with malignant posterior reversible encephalopathy syndrome (PRES) complicated by intracranial hemorrhage (ICH). ⋯ Eclamptic women with malignant PRES and ICH can achieve long-term outcomes comparable to those without hemorrhagic transformation through timely surgical intervention following AHA guidelines, though the natural course of such cases remains unclear.
