Randomized Controlled Trial[Benefit of misoprostol for prevention of postpartum hemorrhage in cesarean section: a randomized controlled trial].
To assess the benefit of sublingual misoprostol in addition to standard oxytocin in the prevention of post-partum hemorrhage at caesarean section. ⋯ Sublingual misoprostol (in addition to oxytocin) is effective in prevention of post-partum hemorrhage at caesarean sections when compared to oxytocin alone, without major side-effects. Larger studies are needed to confirm our results.

Randomized Controlled Trial Comparative Study Clinical Trial[Use of etilefrin as local vasoconstrictor in lower gynecologic surgery ].
In this randomized prospective study the authors have compared the effectiveness and side-effects of two local vasoconstrictor agents, etilefrine (Effortil) and ornithine 8 vasopressin (Por 8) in vaginal gynaecological surgery. Thirty-three patients entered the trial and were divided into two groups: G1 (15 patients) received Effortil, and G2 (18 patients) received Por 8. The products, administered at random, were diluted in saline 40 ml and injected into the cervix through 6 points. ⋯ Diastolic BP was significantly higher in G2 than in G1 (P less than 0.002, Fisher test). Systolic BP was also elevated in that group (P less than 0.03, chi 2 test). Moderate reduction in heart rate was observed in both groups (P less than 0.3, Fischer test), but severe (48 beats/min) bradycardia was noted in one G2 patient.

Review Case Reports[Endometriosis with massive hemorrhagic ascites: a case report and review of the literature].
Endometriosis is defined as the presence of endometrial tissue outside the uterine cavity. It generally involves the peritoneum, ovaries and rectovaginal septum. Its characteristic symptoms include dysmenorrhea, pelvic pain, deep dyspareunia and infertility. ⋯ The future treatement will consists of GnRH analog for about six months, which will be relayed by a long term progestative therapy. A diagnosis of endometriosis should always be considered in middle-age women who presents with bloody ascites. Long follow-up is advisable for patients who undergo conservative treatment because of thehigh risk of recurrence.

Review Comparative Study[Effects of maternal analgesia and anesthesia on the fetus and the newborn].
Continuous electronic fetal heart rate (FHR) monitoring is part of routine care for laboring patients under either systemic or locoregional analgesia. Opioid systemic analgesia (mainly meperidine in early labor), yet less frequently used in our country, is associated with a decrease in FHR-variability and worse acid-base and neonatal status compared to epidural or combined spinal epidural analgesia. ⋯ All these undesirable side effects which may induce severe intrapartum fetal distress must be adequately detected and treated with intrauterine resuscitation techniques, including correction of maternal hypotension and/or the use of tocolytics agents. Reinstallation of electronic fetal monitoring at arrival in the operating room before cesarean section for suspected fetal hypoxia may be helpful to choose better anesthetic technique and try to avoid general anesthesia associated with increased maternal morbidity and mortality.

[Respiratory distress syndrome in the adult and pregnancy. Hemodynamic consequences].
RDS in adults is an unusual complication of pregnancy. We analysed a case of adult respiratory distress syndrome during pregnancy and assessed the haemodynamic data under controlled ventilation whilst a spontaneous delivery was carried out. The rapid drop in pulmonary artery pressure and in the QS-QT complex after the delivery suggests that there may be a placental shunt which gives a too high reading for exaggeration of the left to right intrapulmonary shunt figures obtained.
