To investigate differences in the quality of emergency care for children related to differences in hospital setting, physician training, and demographic factors. ⋯ The quality of care provided to children is associated with age, hospital setting, and physician training.

Randomized Controlled Trial Multicenter StudyInfliximab treatment of intravenous immunoglobulin-resistant Kawasaki disease.
To investigate the safety, tolerability, and pharmacokinetics of the anti-tumor necrosis factor-alpha monoclonal antibody infliximab in subjects with intravenous immunoglobulin (IVIG)-resistant Kawasaki disease (KD). ⋯ Both infliximab and a second IVIG infusion were safe and well tolerated in the subjects with KD who were resistant to standard IVIG treatment. The optimal management of patients resistant to IVIG remains to be determined.

Multicenter StudyFirst day of life pulse oximetry screening to detect congenital heart defects.
To evaluate the efficacy of first day of life pulse oximetry screening to detect congenital heart defects (CHDs). ⋯ Early pulse oximetry screening promotes early detection of critical CHDs and other potentially severe diseases. The sensitivity rate for detecting critical CHDs is high, and the false-positive rate is low.

Multicenter StudyVariation in inpatient resource utilization and management of apparent life-threatening events.
To report national variations in diagnostic approaches to apparent life-threatening events (ALTEs) and resource utilization. ⋯ There is considerable hospital-based variation in care for patients hospitalized for conditions potentially identifiable as ALTE, particularly in the evaluation and diagnosis of gastroesophageal reflux, which may contribute to adverse clinical and financial outcomes. An evidence-based national standard of care for ALTE is needed, as are multi-institutional initiatives to study different diagnostic and management strategies and their effect on patient outcomes.

Randomized Controlled Trial Multicenter StudyTherapeutic hypothermia changes the prognostic value of clinical evaluation of neonatal encephalopathy.
To evaluate whether therapeutic hypothermia alters the prognostic value of clinical grading of neonatal encephalopathy. ⋯ Infants with moderate encephalopathy on day 4 may have a more favorable prognosis after hypothermia treatment than expected after standard care.
