Multicenter StudyImproving safety culture on adult medical units through multidisciplinary teamwork and communication interventions: the TOPS Project.
The goal of this project was to improve unit-based safety culture through implementation of a multidisciplinary (pharmacy, nursing, medicine) teamwork and communication intervention. ⋯ While it is difficult to isolate the effects of the team training intervention from other events occurring during the year between training and postevaluation, overall the intervention seems to have improved the safety culture on these medical units.

Multicenter StudyWeekend mortality for emergency admissions. A large, multicentre study.
Several studies have identified higher mortality for patients admitted as emergencies at the weekend compared with emergency admissions during the week, but most have focused on specific conditions or have had a limited sample size. ⋯ This is the largest study published on weekend mortality and highlights an area of concern in relation to the delivery of acute services.

Multicenter StudyAdverse events and potentially preventable deaths in Dutch hospitals: results of a retrospective patient record review study.
This study determined the incidence, type, nature, preventability and impact of adverse events (AEs) among hospitalised patients and potentially preventable deaths in Dutch hospitals. ⋯ The incidence of AEs, preventable AEs and potentially preventable deaths in the Netherlands is substantial and needs to be reduced. Patient safety efforts should focus on surgical procedures and older patients.

Multicenter StudyIntensivist physician staffing and the process of care in academic medical centres.
Although intensivist physician staffing is associated with improved outcomes in critical care, little is known about the mechanism leading to this observation. ⋯ High intensity physician staffing is associated with increased use of evidence-based quality indicators in patients receiving mechanical ventilation.

Multicenter StudySafety of telephone triage in general practitioner cooperatives: do triage nurses correctly estimate urgency?
In recent years, there has been a growth in the use of triage nurses to decrease general practitioner (GP) workloads and increase the efficiency of telephone triage. The actual safety of decisions made by triage nurses has not yet been assessed. ⋯ Telephone triage by triage nurses is efficient but possibly not safe, with potentially severe consequences for the patient. An educational programme for triage nurses is recommended. Also, a direct second safety check of all cases by a specially trained GP telephone doctor is advisable.
