Randomized Controlled Trial Multicenter Study[Direct total body CT scan in multi-trauma patients].
Immediate total body computed tomography (CT) scanning has become important in the early diagnostic phase of trauma care because of its high diagnostic accuracy. However, literature provides limited evidence whether immediate total body CT leads to better clinical outcome then conventional radiographic imaging supplemented with selective CT scanning in trauma patients. The aim of the REACT-2 trial is to determine the value of immediate total body CT scanning in trauma patients. ⋯ The REACT-2 trial is the first multicenter randomized clinical trial that will provide evidence on the value of immediate total body CT scanning during the primary survey of severely injured trauma patients.

Randomized Controlled Trial[Prevention of nosocomial infections after cardiac surgery by decontamination of the nasopharynx and oropharynx with chlorhexidine; a prospective, randomised study].
To determine the efficacy ofperioperative decontamination of the nasopharynx and oropharynx in reducing nosocomial infection after cardiac surgery with the use of 0.12% chlorhexidine. ⋯ Decontamination of the nasopharynx and oropharynx with chlorhexidine appeared to be an effective method to reduce nosocomial infection after cardiac surgery.

Randomized Controlled Trial[Less ventilator-associated pneumonia after oral decontamination with chlorhexidine; a randomised trial].
To determine the effect of oral decontamination with either chlorhexidine (CHX, 2%) or the combination chlorhexidine-colistin (CHX-COL, 2%-2%) on the frequency and the time to onset of ventilator-associated pneumonia in Intensive Care patients. ⋯ Oral decontamination of the oropharyngeal cavity with chlorhexidine or the combination chlorhexidine-colistin reduced the incidence and the time to onset ofventilator-associated pneumonia.

Randomized Controlled Trial[Fewer lung complications following inspiratory muscle training in patients undergoing coronary bypass surgery: a randomized trial].
The aim of this study was to evaluate the prophylactic efficacy of preoperative physiotherapy, including inspiratory muscle training (IMT), on the incidence of postoperative pulmonary complications (PPCs) in high-risk patients scheduled for elective coronary artery bypass grafting (CABG). ⋯ Preoperative physiotherapy, including IMT, statistically significantly reduced the incidence ofPPCs and the duration ofpostoperative hospitalization in patients at high risk of developing a pulmonary complication on undergoing CABG.

Randomized Controlled Trial[Fracture prevention in postmenopausal women with osteoporosis by an annual infusion of zoledronic acid].
A randomised, placebo-controlled study into the effects of the annual intravenous administration of zoledronic acid for the prevention of fractures in postmenopausal women with osteoporosis was recently published. Zoledronic acid, a bisphosphonate, inhibits osteoclasts. The study included 7765 patients with osteoporosis, between the ages of 65 and 89 years, who were given calcium and cholecalciferol daily by mouth in addition to 5 mg zoledronic acid or placebo once a year. ⋯ Fewer fractures were seen in the study group than in the control group: after 3 years, hip fractures were reduced by 41%, non-vertebral fractures by 25% and vertebral fractures by 70%, already within one year (by 6o%). Side effects included acute phase reactions, atrial fibrillation, ocular complaints, renal-function disorders, and hypocalcaemia. Some examples of candidates for treatment with zoledronic acid would be patients with osteoporosis in whom oral treatment meets objections or who are hospitalised due to a fracture.
