Randomized Controlled Trial Comparative Study Clinical Trial
A randomized trial of non-synchronized Nasopharyngeal Intermittent Mandatory Ventilation (nsNIMV) vs. Nasal Continuous Positive Airway Pressure (NCPAP) in the prevention of extubation failure in pre-term < 1,500 grams.
To compare the rate of reintubation within 7 days after extubation and study the complications in premature infants who were randomized in the immediate postextubation period to either nsNIMV or NCPAP. ⋯ Non-invasive mode of ventilation, both NIMV and NCPAP, for weaning ofpre-term infants from ventilator may reduce the rate of reintubation in this group. Both modes seem to be equally safe. We believe that the use of non-invasive ventilator techniques will significantly reduce neonatal morbidity in the future. Additional prospective evaluation of these approaches should be conducted in the future.

Effect of scalp infiltration on postoperative pain relief in elective supratentorial craniotomy with 0.5% bupivacaine with adrenaline 1:400,000.
Determine the effect of scalp infiltration on postoperative craniotomy pain with 0.5% bupivacaine with adrenaline 1:400,000. ⋯ Surgical wound infiltration, before skin closure, with 0.5% bupivacaine with adrenaline decreased the incidence and severity of postoperative pain in patients undergoing supratentorial craniotomy, but only for the first hour after surgery.

A comparison of oral chloral hydrate and sublingual midazolam sedation for echocardiogram in children.
To compare the efficacy and safety of oral chloral hydrate and sublingual midazolam to sedate the children undergoing echocardiography. ⋯ Sublingual midazolam at the dose of 0.3 mg/kg can be used to sedate the children at age group between 6 months to 5 years who undergoing echocardiogram with comparable rate of success and safety as 50 mg/kg of chloral hydrate orally. The less depth in the level of consciousness after sedation with midazolam compare to chloral hydrate may be advantage in a high risk patient to avoid deep sedation but may be disadvantage in case who need more comprehensive echocardiographic evaluation.

Comparison on the efficacy of dexpanthenol in sea water and saline in postoperative endoscopic sinus surgery.
To compare the efficacy of dexpanthenol spray and saline irrigation in the postoperative care of sinusitis patients following endoscopic sinus surgery (ESS). ⋯ The efficacy of dexpanthenol was comparable to nasal saline irrigation in the postoperative care of sinusitis patients following endoscopic sinus surgery. Dexpanthenol is an alternative treatment, which may be useful in young children and complicated cases.

Choice of colloidal solutions in dengue hemorrhagic fever patients.
DHF is characterized by plasma leakage and abnormal hemostasis. About 20% of DHF patients do require colloidal solution in addition to conventional crystalloid solution for the treatment. There is only one colloidal solution, 10% Dextran-40 in NSS that proved to be effective for this group of DHF patients. ⋯ 10% Haes-steril is as effective as 10% dextran-40 in the treatment of DHF patients who have severe plasma leakage. There are no differences in DHF disease severity and complications in both groups but the disease severity and complications, especially fluid overload are observed to be more comparative with admitted DHF patients. Both colloidal solutions are safe in DHF patients with no allergic reaction observed and no interference in renal functions and hemostasis.
