Randomized Controlled Trial[Study the effects of high-volume hemofiltration and fluid resuscitation on removing blood lactic acid and pro-inflammatory cytokines in patients with refractory septic shock].
To evaluate the effects of high-volume hemofiltration (HVHF) and fluid resuscitation on the levels of arterial lactate and inflammatory cytokines, the acute physiology and chronic health evaluation II (APACHE II) score, and sepsis-related organ failure assessment (SOFA) score in patients with refractory septic shock and multiple organ dysfunction syndrome (MODS). ⋯ HVHF and fluid resuscitation could reduce arterial lactate and cytokines contents, at the same time lower the APACHE II score and SOFA score in patients with refractory septic shock and MODS, thus it could improve the survival rate of MODS patients.

Randomized Controlled Trial[Randomized control study of sequential non-invasive following short-term invasive mechanical ventilation in the treatment of acute respiratory distress syndrome as a result of existing pulmonary diseases in elderly patients].
To evaluate the feasibility and the efficacy of sequential non-invasive mechanical ventilation (MV) following short-term invasive MV in the treatment of acute respiratory distress syndrome (ARDS) consequent to pulmonary diseases in the elderly patients. ⋯ Early extubation followed by non-invasive ventilation might decrease the total duration of ventilation support, the risks of VAP and duration of RICU stay.

[A study of cough peak expiratory flow in predicting extubation outcome].
To study the value of cough peak expiratory flow (CPEF) in predicting extubation outcome of patients. ⋯ After patients have recovered from respiratory failure and SBT is successful, factors affecting airway competence, such as cough strength, may be important predictors of extubation outcome. The study confirms that CPEF is a strong and independent predictor of extubation outcome when the patient is mentally clear and has a successful SBT. When the CPEF>58.5 L/min, the successful rate is high. On the contrary, when the CPEF< or =58.5 L/min, the unsuccessful rate is high. Failure is often due to poor cough. The extubation outcome is highly correlated with cough strength. The prognosis in patients with failure is poor.

[Early enteral nutrition and gastric intramucosal pH monitoring in patients with mechanical ventilation].
To observe the effect of early enteral nutrition and the significance of gastric intramucosal pH (pHi) monitoring in patients without gastrointestinal trauma or surgery requiring mechanical ventilation in the intensive care unit (ICU). ⋯ Early enteral nutrition can improve the blood perfusion and oxygenation of gastrointestinal mucosa, and the success rate of weaning of mechanical ventilation, help reduce complications, and shorten the mechanical ventilation duration. Enteral nutrition support should be given to ICU patients with mechanical ventilation as early as possible provided that the enteral function is normal.
