ReviewParoxysmal nocturnal hemoglobinuria: advances in the understanding of pathophysiology, diagnosis, and treatment.
In recent years, "old" paroxysmal nocturnal hemoglobinuria (PNH) has achieved new advances in terms of the understanding of its pathophysiology, modern approach to diagnostics, optimization of therapy, and dynamic development of new therapeutic agents. This review emphasizes the greater than previously recognized importance of the reduced susceptibility of PNH stem cells to apoptosis in the selection of a defective clone. Some changes in cytokine and chemokine profiles in patients with PNH have been interpreted in the context of autoimmunity and apoptosis. ⋯ Determinants of modern treatment, such as strategies (complement C5 inhibitors vs hematopoietic stem cell allotransplantation), the safety and efficacy of treatment with eculizumab or ravulizumab, policy of initiation and monitoring of treatment, the criteria for response to treatment and final outcomes of treatment are described. Among the new therapeutic agents, crovalimab and C5 inhibitors at a less advanced stage of research are discussed: tesidolumab, pozelimab, zilucoplan, nomacopan, and cemdisiran. The first approved proximal complement pathway inhibitors that primarily prevent extravascular hemolysis, pegcetacoplan, danikopan, and iptacopan, are presented and their potential benefits are highlighted.

Multicenter StudyEfficacy and safety of antazoline for cardioversion of atrial fibrillation: propensity score matching analysis of multicenter registry (CANT II Study).
Due to safety concerns about available antiarrhythmic drugs (AADs), reliable agents for termination of atrial fibrillation (AF) are requisite. ⋯ Antazoline appears to be an efficacious agent for termination of AF in real‑world setting. Randomized controlled trials are required to evaluate its safety in specific patient populations.

Observational StudyPost-COVID-19 complications in hospitalized and non-hospitalized patients: the Silesian complications of the COVID-19 (SILCOV-19) database.
Due to the extent of the pandemic, high prevalence and severity of complications in the early post‑recovery period are expected. ⋯ The abnormal platelet parameters, functional and radiological findings in the lungs, and insomnia were the most frequent short‑term COVID‑19 complications in hospitalized and nonhospitalized patients. Considering the number of patients who have had COVID‑19 worldwide, a high burden of the post-COVID‑19 complications might be expected.

Post-discharge pneumonia after hip arthroplasty. A population-based study of 55,842 patients in Poland.
Alloplasty is one of the most frequently performed procedures, as it hugely improves the quality of life. ⋯ Pneumonia is one of the most common postoperative infections after hip endoprosthesis, especially in winter, requiring rehospitalization. Efforts should be made to prepare patients in the perioperative period.

Continuous positive airway pressure treatment reduces ventricular arrhythmias in obstructive sleep apnea patients with nocturnal dominance of arrhythmias and in those with severe desaturations.
Continuous positive airway pressure (CPAP) treatment is considered effective in reducing ventricular arrhythmias (VAs) in patients with obstructive sleep apnea (OSA). ⋯ CPAP treatment decreases VAs in OSA patients, especially those with severe desaturations and nocturnal domination of PVCs. Night / day PVC ratio might be a useful clinical parameter predicting reduction of PVCs in these patients.
