Case Reports[Case report of disseminated carcinomatosis of the bone marrow originating from gastric cancer with cancer-related disseminated intravascular coagulation successfully treated with recombinant human soluble thrombomodulin].
A 62-year-old man, who underwent distal gastrectomy due to gastric cancer, was diagnosed with disseminated carcinomatosis of the bone marrow 8 years later. Chemotherapy was administered following treatment with recombinant human soluble thrombomodulin (rTM), and as a result, he successfully recovered from his disseminated intravascular coagulation (DIC) status and experienced improvement of his severe cancer-related pain. The use of rTM may enable the safe continuation of chemotherapy, and rTM may also be a useful treatment for DIC associated with solid cancer, such as gastric cancer.

[Heavy ion radiotherapy for prostate cancer -- the role of hormone therapy].
The role of androgen deprivation therapy combined with heavy ion radiotherapy involving carbon ions was reviewed. In conventional radiotherapy, short- and long-term androgen deprivation therapies were recommended for patients with intermediate- and high-risk prostate cancers, respectively. ⋯ Heavy ion radiotherapy and androgen deprivation therapy were performed in combination according to risk criteria. Further studies are warranted to determine the optimal method and duration of androgen deprivation therapy during heavy ion radiotherapy for prostate cancer.

Of late, there has been rapid development of chemotherapeutic agents for treating metastatic colorectal cancers. However, the so-called "druglag" is a long-standingproblem; it refers to the drugapproval delays in Japan that occur after drugs have been developed and approved in Europe and the USA. ⋯ Data from the global phase III RECOURSE trial were presented in the ESMO-GI 2014, where the efficacy of TAS-102 was proved again. Herein, we present data about the efficacy and side effects of TAS-102 from each clinical trial.

Case Reports[A case of curative resection after downsizing chemotherapy in initially unresectable locally advanced intrahepatic cholangiocarcinoma].
This case report describes an 83-year-old man with intrahepatic cholangiocarcinoma who was referred by a local hospital. Abdominal computed tomography (CT) showed a large tumor in hepatic segments 4, 5, and 8 involving the right hepatic vein and inferior vena cava, which is normally indicative of an unresectable locally advanced tumor. After systemic chemotherapy with gemcitabine and cisplatin, the observed decrease in the level of tumor marker suggested that the cancer was responding to treatment, while radiological findings showed the main tumor shrunk without the presence of distant metastases. ⋯ Eighteen months after surgery, the patient is free of disease and shows no signs of recurrence. An initially unresectable, locally advanced biliary tract cancer may be down sized by chemotherapy, which makes radical resection possible, at least in a proportion of patients. This approach provides longer survival and may have a potential for disease eradication as a new multidisciplinary approach for patients with unresectable locally advanced biliary tract cancer.

Case Reports[Efficiency of pre-operative chemoradiotherapy in treating a case of advanced rectal cancer].
There is insufficient evidence for the pre-operative use of chemoradiotherapy (CRT) in treatment of advanced rectal cancers, and its efficiency and safety are unclear. However, it has recently been suggested that a new class of carcinostatic agents are more effective during preoperative CRT. ⋯ The Japanese Society for Cancer of Colon and Rectum (JSCCR) guidelines for 2014 also recommend the use of both drugs during preoperative CRT. We report a case of rectal cancer, which was successfully treated with radical resection and neoadjuvant chemoradiotherapy.
