Review Case Reports[A fatal case of acute mediastinitis caused by periodontal infection].
A 57-year-old man had suffered from poorly controlled diabetes mellitus and liver cirrhosis due to alcohol and hepatitis C for about 10 years. He developed fever and swelling of the right cheek and neck due to periodontal infection. The symptoms worsened in spite of antibiotic therapy and were accompanied by dyspnea. ⋯ However, renal and liver dysfunction developed and the patient died of multiorgan failure after 35 days of hospitalization. Death due to periodontal infection is rare. We give a review of the literature.

Review Case Reports[A case of acute eosinophilic pneumonia with acute onset of dyspnea].
A 16-year-Old boy was admitted with acute onset of fever, cough, and dyspnea. Chest radiography showed diffuse infiltrated shadow and right pleural effusion. BALF revealed a high level of eosinophils (56.8%), and TBLB specimen showed infiltration of inflammatory cells, chiefly eosinophils. ⋯ Acute eosinophilic pneumonia was diagnosed as in the case reported by Allen et al. However, the present patient had a high IgE level and history of atopic allergic rhinitis. In more than one third of previously reported cases in Japan, there was a history of atopy, unlike in Allen's report.

Review Case Reports[Case report and review of literature on seasonal distribution and pathogenesis of acute eosinophilic pneumonia in Japan].
A 49-year-old female was admitted because of high grade fever and dyspnea. Chest roentgenogram revealed diffuse reticulo-linear shadows. Laboratory findings showed peripheral blood eosinophilia, and blood gas analysis demonstrated severe hypoxemia. ⋯ Eleven of 22 cases were tested for precipitating antibodies to several fungi, 3 of which showed positive results against Trichosporon cutaneum, Trichoderma viride and Aspergillus species. The clinical features of acute eosinophilic pneumonia resemble those of summer type hypersensitivity pneumonitis. Therefore, it is important to diagnose AEP on the basis of clinical symptoms, and precipitating antibody, viral titer and pathological findings.

Review Case Reports[A case of endobronchial chondroma in a patient with atypical mycobacterial infection].
A case of endobronchial chondroma in a 74-year-old man with atypical mycobacterial infection is reported. Bronchoscopy was performed in order to exclude the possibility of malignancy. At bronchoscopic examination, a hemispherical polypoid tumor with smooth surface was found at the orifice of left B3 and was removed by transbronchial biopsy forceps. ⋯ About 1 year after forceps resection, no growth of the tumor was seen in the left upper lobe bronchus on bronchoscopic examination. Endobronchial chondroma is relatively rare. We discuss the clinical features of 19 cases of endobronchial chondroma so far reported in Japan, including our case.

Review Case Reports[Mediastinal esophago-bronchogenic cyst presenting as a single mass].
An abnormal shadow was found on a chest X-ray film of a 42-year-old man. The mass was 17 x 12 x 11 cm, smooth, round, homogeneous, and was seen in the left upper lung field. Chest computed tomography revealed a cystic mass in contact with the left lung, the chest wall, the esophagus, and the aorta. ⋯ The final diagnosis was mediastinal esophago-bronchogenic cyst. In previously reported cases, two cystic masses were connected by a canal. The findings in this case support the theory by Yoshii that this type of cyst originates from the diverticulum of the foregut near the lung bud.
