Case Reports[Assessment of the permeability of the pulmonary microvasculature using radiotracers in a case of adult respiratory distress syndrome].
A 48-year-old man with small cell lung cancer developed ARDS, and massive pulmonary edema fluid was obtained with the fiberoptic bronchoscopy. The pulmonary edema fluid to serum ratios of total protein and albumin were 0.72 and 0.85 respectively. The ratio of LDH was higher (2.71), while that of cholesterol was lower (0.11) than that of total protein. ⋯ The time activity curves of I-123 IMP and I-131 HSA in his blood samples revealed almost constant radioactivity from 5 minutes to 120 minutes after injection, while both radioactivity levels in pulmonary edema fluid samples increased with time. The clearance ratio of I-123 IMP to I-131 HSA was constant at each sampling time (mean +/- SD, 1.51 +/- 0.32). The linear correlation between I-123 IMP clearance and I-131 HSA clearance (r = 0.95, p less than 0.01) suggested that the clearance ratio of exudative plasma components may remain unchanged even if pulmonary microvasculature permeability has changed.

Case Reports[A case of bronchial actinomycosis associated with bronchoscopically removed broncholith].
A 57-year-old woman with bronchial actinomycosis associated with broncholith is presented. She had suffered from fever and productive cough for 7 years. Her chest roentgenograms showed atelectasis of the middle lobe and calcification in the middle lobe bronchus. ⋯ After treatment with antibiotics, the broncholith became movable, and it was removed bronchoscopically. Its shape was like a molding of the right middle lobe bronchus. We speculate that the growth of this broncholith partially resulted from chronic inflammation associated with actinomycelial infection.

Case Reports[Cervical lung hernia in a case of severe chronic asthma and bronchitis].
A case of a bilateral pulmonary hernia of the lungs into the supraclavicular fossae is described. A man, aged 79, with severe chronic asthma and chronic bronchitis due to air pollution, complained of a cervical mass off and on for several years. ⋯ Radiologically this herniation could best be identified on lateral cervical roentgenogram. It is said that cervical lung hernia is a rare condition, but in our experience supraclavicular herniation of the lung is not unusual in cases of severe chronic obstructive lung disease.

Case Reports[Massive hemoptysis in a patient with a tuberculous thoracic aneurysm].
A 91-year-old man with a history of pulmonary tuberculosis 20 years earlier came to our hospital because of hemoptysis in May 1993. A chest X-ray film showed infiltrative shadows in the left first arch of the cardiac silhouette, and sputum cultures were positive for Mycobacterium tuberculosis. Despite treatment with antituberculosis drugs, chest X-ray films revealed gradual progression, and he had occasional hemoptysis. ⋯ Sudden and massive hemoptysis occurred in January 1994. At autopsy, the wall of the aneurysm and the surrounding tissue were found to have histological changes characteristic of tuberculosis. Tuberculous thoracic aneurysms are quite rare, but may develop in patients with tuberculosis in other areas.

[Lung preserving operation for bronchogenic carcinoma--segmentectomy and bronchoplastic lobectomy].
The results of segmentectomy and bronchoplastic lobectomy for bronchogenic carcinoma were reported. Until December 1992, 920 patients underwent lung resection for primary lung cancer at our institution. Among them, 70 patients (7.6%) had segmentectomy and 149 (16.2%) had bronchoplastic lobectomy. ⋯ Five-year survival rates of those with bronchoplasty were 33.9% in total, 51.1% in those with Stage I, 29.1% in those with Stage IIIA and 39.0% in those with squamous cell carcinoma. Respiratory function was well preserved in patients with segmentectomy and bronchoplastic lobectomy. Lung-preserving operations such as segmentectomy and bronchoplastic lobectomy are meaningful as a curative procedure for primary lung cancer for selected patients.
