Review[Questionable documentation of the effect of pregabalin in fibromyalgia].
Use of Pregabalin has increased considerably in Norway during the last years. One explanation is prescription to patients with diffuse musculo-skeletal pain. The aim of the article is to clarify the effect of pregabalin in fibromyalgia. ⋯ Recommendations for pregabalin in treatment of patients with fibromyalgia are based on rather weak evidence. Until trials independent of industry-funding are published, the role of pregabalin in treatment of fibromyalgia remains unclear.

Review[Patent foramen ovale and decompression illness in divers].
About 25 % of the population has patent foramen ovale, and the condition has been assumed to be a causal factor in decompressive illness. Transcatheter closure is possible and is associated with a relatively low risk, but it has not been clarified whether there is an indication for assessment and treatment of the condition in divers. The present study explored a possible relationship between a patent foramen ovale and the risk for decompression illness in divers, if there are categories of divers that should be screened for the condition and what advice should be given to divers with this condition. ⋯ We do not recommend screening for patent foramen ovale in divers because the absolute risk of decompression illness is small and transcatheter closure is only indicated after decompression illness in some occupational divers.

Regional anaesthesia (epidural or spinal anaesthesia) is increasingly used for elective and emergency caesarean sections and is considered to be safer for the mother than general anaesthesia. We present some factors that are important to consider when choosing the anaesthetic method for caesarean section. ⋯ There are few adverse effects of regional anaesthesia in caesarean section except for hypotension that can be easily treated.

Paediatric anaesthesia comprises newborn, infants and children, necessitating a variety of techniques and approaches. ⋯ Paediatric anaesthesia presents special challenges. Safe practice requires experience and knowledge of age-related anatomy, physiology and pathology.

Review[Treatment of acute pain during drug-assisted rehabilitation].
Patients on drug-assisted rehabilitation have the same right to pain relief as others. Techniques that reduce the need for opioids should be used when possible in opioid-dependent individuals who need treatment of acute and post-operative pain. ⋯ Higher doses of opioids than those needed in other patients may be required for analgesia. Well-designed clinical studies are lacking in this field.
