Randomized Controlled Trial Comparative StudyComparison of lidocaine suppositories and periprostatic nerve block during transrectal prostate biopsy.
The aim of this randomized prospective and partially double-blind study was to evaluate the efficacy of transrectal lidocaine applied as suppositories in comparison to periprostatic infiltration as methods of reducing pain during transrectal prostate biopsy. 100 patients were randomized to four groups and received either a suppository containing 60 mg of lidocaine 2 h before biopsy, a 120-mg lidocaine suppository 1 h before biopsy, a 120-mg lidocaine suppository 2 h before biopsy, or they were anaesthetized with a periprostatic infiltration of 5 ml 2% lidocaine. In all patients the same 10-core transrectal biopsy technique was performed. Pain was evaluated using a visual pain scale ranging from 0 to 10 points. ⋯ No patient had severe pain. Eight of the 9 patients with no pain were in the periprostatic injection group. Thus, all lidocaine suppositories showed a good analgesic effect although a significantly better pain reduction was achieved by periprostatic lidocaine infiltration.

Case ReportsMalignant epithelioid renal angiomyolipoma involving the inferior vena cava in a patient with tuberous sclerosis.
Renal angiomyolipomas (AMLs) are mesenchymal tumors that occur either sporadically or are associated with tuberous sclerosis, and are generally considered to be benign. Malignant AML is extremely rare, and most are found to be epithelioid histopathologically. The authors report the case of a patient followed for renal AML. ⋯ The patient was treated by nephrectomy and tumor thrombectomy with retroperitoneal lymph node dissection. Histological examination demonstrated renal AML with a malignant epithelioid contingent. The various aspects of this histological and radiological variant are discussed.

Inadequacy of prostate-specific antigen doubling time estimates calculated using an ultrasensitive assay of prostate-specific antigen for biochemical failure after radical prostatectomy.
The objective of this study was to evaluate the adequacy in calculating prostate-specific antigen doubling time (PSADT) using an ultrasensitive assay of prostate-specific antigen (PSA) for biochemical failure after radical prostatectomy (RP). ⋯ These findings suggest that calculating PSADT using an ultrasensitive assay for biochemical failure after RP may still be open for discussion.

To evaluate the effects of periprostatic bupivacaine administration on pain control and analgesic consumption after transurethral prostate resection (TURP). ⋯ Periprostatic bupivacaine administration was a useful and safe method for postoperative pain control and reduced analgesic consumption in patients undergoing TURP.

Comparative Study Clinical TrialImpact of additional sampling in the TRUS-guided biopsy for the diagnosis of prostate cancer.
To evaluate the diagnostic value of 10+ systematic sampling technique when performing transrectal ultrasound-guided (TRUS) prostate biopsy, compared with the sextant biopsy technique for patients with suspected prostate cancer. ⋯ The 10+ systematic TRUS-guided prostate biopsy improves the detection rate of prostate cancer compared to the sextant biopsy technique alone, especially when performed in men with positive PSA, PSA ratio, and negative DRE.
