Randomized Controlled Trial Multicenter StudyA comparison of insulin injection needles on patients' perceptions of pain, handling, and acceptability: a randomized, open-label, crossover study in subjects with diabetes.
For some diabetes patients, subcutaneous injections are associated with pain, discomfort, and anxiety. We compared overall preferences of two needle types: NovoFine((R)) 32-gauge tip x 6 mm and NovoFine 30-gauge x 8 mm, when used with the FlexPen((R)) injection device (all from Novo Nordisk A/S [Copenhagen, Denmark] and Novo Nordisk Ltd. [Crawley, UK]). ⋯ NovoFine 32-gauge tip x 6 mm needles are a viable alternative to NovoFine 30-gauge tip x 8 mm needles for people administering daily insulin injections.

Randomized Controlled Trial Comparative StudyPatient perceptions of injection pain and anxiety: a comparison of NovoFine 32-gauge tip 6mm and Micro Fine Plus 31-gauge 5mm needles.
Injecting insulin may cause pain and anxiety and lead to suboptimal treatment of diabetes mellitus. The aim of this randomized, open-label, crossover study was to compare two types of needle design and diameter in patients with diabetes injecting insulin. ⋯ These results suggest that the diameter and design of the needle play an important role in reducing injection pain and needle anxiety, which is particularly important for insulin initiation.

Randomized Controlled TrialPre- and postoperative accuracy and safety of a real-time continuous glucose monitoring system in cardiac surgical patients: a randomized pilot study.
Our objective was to evaluate the accuracy and safety of a real-time (RT) continuous glucose monitoring system (CGMS) in patients before and after cardiothoracic surgery and to investigate whether activation of the alarm function of the RT-CGMS had an effect on glucose control. ⋯ Although the RT-CGMS is safe and accurate according to accepted standards, there are still small aberrations, which in our opinion preclude unlimited use in its present form in a clinical setting. The effect of the alarm function at different glucose levels remains to be investigated.

Randomized Controlled Trial Multicenter Study Clinical TrialThe incretin mimetic exenatide as a monotherapy in patients with type 2 diabetes.
Exenatide is an adjunctive therapy for type 2 diabetes, and preliminary evidence suggests that its glucoregulatory effects may be similar in the absence of oral therapy. ⋯ Exenatide twice-daily monotherapy resulted in glycemic improvements and reductions in body weight comparable to that of exenatide combination therapy with metformin in patients with type 2 diabetes.

Randomized Controlled Trial Comparative StudyUsability of a microtapered needle (TN3305) for insulin treatment in japanese patients with diabetes mellitus: a comparative clinical study with a standard thin wall needle.
We evaluated the clinical usability of a microtapered needle (TN3305, "Needle T," Terumo Corp., Tokyo, Japan) by comparing it with a standard needle (Micro-Fine +, 31 gauge, 5 mm, thin wall, "Needle B," Nippon Becton Dickinson Co., Ltd., Tokyo) in a multicenter study. ⋯ The preferences and VAS scores suggested that Needle T caused less pain than Needle B. The incidence of adverse events and safety were equivalent for both needles. We believe that usability of Needle T by patients for self-injection is better.
