Letter Case ReportsCombined central retinal artery and vein occlusion from orbital inflammatory pseudotumour.
Progressive, painful blurred vision and proptosis developed in the left eye of a 32-year-old man over an 8-day period. On initial neuro-ophthalmic evaluation the visual acuity in the left eye was no light perception. Erythema of the periorbital skin, 5 mm of proptosis, and ophthalmoplegia were noted on the left side. ⋯ The diagnosis of orbital inflammatory pseudotumour was made and the orbital signs improved rapidly with 80 mg of prednisone per day. Combined occlusion of the central retinal artery and vein is a rare complication of orbital inflammatory pseudotumour. It is postulated that marked distension of the optic nerve caused mechanical compression of the central retinal vessels leading to the vascular occlusions.

Randomized Controlled Trial Comparative Study Clinical TrialSub-Tenon infiltration or classical analgesic drugs to relieve postoperative pain.
In a prospective randomized double-blind study, the analgesic effect produced by sub-Tenon infiltration was compared with classic analgesic drugs in patients scheduled for posterior segment surgery under general anaesthesia. ⋯ Sub-Tenon infiltration with 3 mL of bupivacaine 0.50% offers excellent postoperative analgesia for about 6 h and is an excellent alternative to classical drugs. Furthermore, it is highly reliable and safe.

A 45 year old man presented with talc retinopathy that had been present for decades. The static nature of this condition indicates that, in the absence of ongoing intravenous drug use, ophthalmological follow up is not necessary

Eye disease is a huge and largely unrecognized problem in our community, especially in the elderly. The Visual Impairment Project, an intensive epidemiological study conducted by the Centre for Eye Research Australia (CERA) from 1991 to 1999, determined the prevalence, causes and risk factors for eye disease in Australia, and helped identify a series of specific, often simple, steps and strategies to significantly reduce its impact. A distillation of CERA"s findings has highlighted the growing problem of eye disease in our community. ⋯ This partnership was established in response to the Global Initiative to Eliminate Avoidable Blindness by the year 2020 - Vision 2020: the Right to Sight. This Global Initiative was established by the World Health Organization and the International Agency for the Prevention of Blindness. Vision 2020 Australia aims to be the peak body for all those involved in vision care in its broadest sense in Australia, and to put vision on the national agenda.

Randomized Controlled Trial Comparative Study Clinical TrialA comparison of cataract surgery under topical anaesthesia with and without intracameral lignocaine.
To compare the efficacy of intracameral unpreserved lignocaine with placebo during cataract surgery under topical anaesthesia. ⋯ This study found no additional benefit of intracameral unpreserved lignocaine when performing routine clear corneal phacoemulsification under topical anaesthesia.
