Metabolic acidosis with elevated anion gap and euglycemic ketoacidosis in pregnant and postpartum women with severe Covid-19.
Pregnant and postpartum women have a higher risk for developing severe Covid-19 and other clinical and obstetric complications. This study aims to evaluate the frequency of metabolic acidosis with elevated anion gap and describe a case series of euglycemic ketoacidosis (EKA) in pregnant and postpartum women with severe confirmed or suspected Covid-19. ⋯ The prevalence of metabolic acidosis and EKA in pregnant and postpartum women with severe Covid-19 was high. This condition should be routinely, so it can be promptly treated.

We attempted to evaluate the immediate high-grade squamous intraepithelial lesion-cervical intraepithelial neoplasia grade 2/3 or worse (HSIL-CIN2+/3+, hereafter referred to as CIN2+/3+) risk of specific human papillomavirus (HPV) genotype and form the precise risk-based triage strategy for atypical squamous cells of undetermined significance (ASC-US) women. ⋯ A new triage strategy for ASC-US women was successfully constructed based on CIN2+/3+ risks of 14 high-risk and 4 intermediate-risk HPVs, which could significantly reduce unnecessary colposcopies.

To comprehensively investigate the predictive value of thyroid hormone sensitivity parameters for cervical lymph node metastasis in patients diagnosed with differentiated thyroid cancer (DTC) undergoing total thyroidectomy and neck lymph node dissection. ⋯ The findings provide compelling evidence for the predictive value of thyroid hormone sensitivity parameters, particularly T3, T4, Tg, TgAbs and TSH receptor antibody, in identifying and evaluating the likelihood of cervical lymph node metastasis in patients with DTC. These parameters hold potential implications for risk stratification, clinical decision-making and personalized management strategies, contributing to improved outcomes for patients at risk of lymph node metastasis.

This study expolored the relationship between perivascular adipose tissue (PVAT) radiomic features derived from coronary computed tomography angiography (CCTA) and the presence of coronary artery plaques. It aimed to determine whether PVAT radiomic could non-invasively assess vascular inflammation associated with plaque presence. ⋯ CCTA-based PVAT radiomics effectively distinguished coronary artery segments with and without plaques. The combined model and nomogram demostrated clinical utility, offering a novel approach for early diagnosis and risk stratification in coronary heart disease.

The incidence of invasive infection of Klebsiella pneumoniae (Kp) in the community is increasing every year, and the high disability and mortality rates associated with them pose great challenges in clinical practice. This study aimed to explore the clinical and microbiological characteristics of Kp invasive infection in the community. ⋯ Patients with cKp invasive infections exhibit more severe inflammatory responses and a poorer prognosis, necessitating vigilant attention from clinicians. The treatment of cKp invasive infections remains inconclusive between "heavy-handed strikes" and "sensitivity is sufficient". Focusing solely on the liver and lungs while neglecting infection sites outside of these organs can lead to catastrophic results, which should be avoided during treatment.
