To determine whether post-extubation respiratory support via nsNIPPV decreases the need for mechanical ventilation (MV) compared to nasal continuous positive airway pressure (NCPAP) in preterm infants with respiratory distress syndrome (RDS). ⋯ Compared to NCPAP, nsNIPPV appears to be a feasible mode of extubation in preterm infants with significant beneficial effects of reduced duration of NIV support, supplementary oxygen and decreased rates of BPD.

Randomized Controlled Trial Multicenter Study Comparative StudyConservative Versus Liberal Oxygenation Targets for Mechanically Ventilated Patients - a Pilot Multicenter Randomized Controlled Trial.
There are no randomized controlled trials comparing different oxygenation targets for intensive care unit (ICU) patients. ⋯ Our study supports the feasibility of a conservative oxygenation strategy in patients receiving IMV. Larger randomized controlled trials of this intervention appear justified. Clinical trial registered with Australian New Zealand Clinical Trials Registry (ACTRN 12613000505707).

Randomized Controlled TrialNo-sedation during mechanical ventilation: impact on patient's consciousness, nursing workload and costs.
Evidence is growing that less or no-sedation is possible and beneficial for patients during mechanical ventilation. ⋯ Patients receiving no-sedation are more awake during their stay in ICU. There might be a potential economical saving with the use of a 1:1 nurse-patient ratio and no-sedation compared with sedation and a 1:2 nurse-patient ratio.

Randomized Controlled Trial Multicenter StudyCharacteristics and Outcomes of Eligible Non-Enrolled Patients in a Mechanical Ventilation Trial of Acute Respiratory Distress Syndrome.
Patients eligible for randomized controlled trials may not be enrolled for various reasons. Nonenrollment may affect study generalizability and lengthen the time required for trial completion. ⋯ Nonenrollment was common, with approximately one ENE patient for every randomized patient. Our study suggests that enrollment in trials of mechanical ventilation may be associated with improved outcomes compared with standard care and highlights the need for prospective tracking and transparent reporting of ENE patients as part of trial management.
