Randomized Controlled TrialA pilot study of Biotene OralBalance® gel for oral care in mechanically ventilated preterm neonates.
Neonatal ventilator-associated pneumonia (VAP) is associated with increased morbidity and mortality. In adults on mechanical ventilation, timed oral care decreases the frequency of VAP, but this approach has not been studied in neonates. ⋯ The results of this pilot study support the feasibility of a randomized trial of timed oral care with Biotene OralBalance® gel for prevention of VAP in mechanically ventilated neonates.

Case ReportsManagement of acute respiratory distress syndrome in a French Bulldog using airway pressure release ventilation.
To describe the successful clinical management of a dog with acute respiratory distress syndrome (ARDS) using airway pressure release ventilation (APRV). ⋯ This is the first reported use of APRV to manage refractory hypoxemia associated with ARDS in a dog. This alternative mode of mechanical ventilation can be considered a feasible alternative in canine patients with ARDS.

To compare different nasal cavity nursing methods on mechanically ventilated patients. ⋯ For mechanically ventilated patients, nasal nursing can't be ignored and the new atomizing nasal cleaning is an effective method for VAP prevention.

Tidal volume (V(T)) must be accurately delivered by anaesthesia ventilators in the volume-controlled ventilation mode in order for lung protective ventilation to be effective. However, the impact of fresh gas flow (FGF) and lung mechanics on delivery of V(T) by the newest anaesthesia ventilators has not been reported. ⋯ Under experimental conditions, the volume error differed significantly between the four new anaesthesia ventilators tested and was influenced by FGF, although this effect may not be clinically relevant.

Severe trauma of the chest wall: surgical rib stabilisation versus non-operative treatment.
Serial rib fractures and flail chest injury can be treated by positive-pressure ventilation. Operative techniques reduce intensive care unit (ICU) stay, overall costs, mortality and morbidity, as well as pain. The aim of this study was to evaluate the benefit of surgical rib stabilisation in comparison to non-operative treatment in patients with severe trauma of the chest wall. ⋯ Operative rib stabilisation with plates is a safe therapy option for severe trauma of the chest wall. Provided that the duration of preoperative mechanical ventilation and time spent in the ICU is minimised due to early operation, our data suggest that the stabilisation of serial rib fractures and flail chest may lead to a reduced time of mechanical ventilation, time in the ICU and mortality.
