Electroencephalography (EEG) is playing an increasingly important role in the management of comatose patients in the intensive care unit. ⋯ Applications of continuous EEG to clinical problems are discussed. The most useful role of CEEG appears to be the detection and management of nonconvulsive seizures. There is a need for controlled studies to assess the role for CEEG in neuro-ICUs and general ICUs.

We assessed in eight dogs the accuracy with which electrical impedance tomography (EIT) can monitor changes in lung volume by comparing the changes in mean lung conductivity obtained with EIT to changes in esophageal pressure (Pes) and to airway opening pressure (Pao) measured after airway occlusion. The average volume measurement errors were determined: 26 ml for EIT; 35 ml for Pao; and 54 ml for Pes. Subsequently, lung inflation due to applied positive end-expiratory pressure was measured by EIT (delta VEIT) and Pao (delta VPAO) under both inflation and deflation conditions. ⋯ The average differences between EIT estimates and delta VEIT,occl were 5.8 +/- 44 ml for delta VEIT,vent and 49.5 +/- 34 ml for delta VEIT,occl. The average volume increase for all dogs was 442.2 ml. These results show that EIT can provide usefully accurate estimates of changes in lung volume over an extended time period and that the technique has promise as a means of conveniently and noninvasively monitoring lung hyperinflation.

Vectorcardiographic changes as predictors of cardiac complications during major vascular surgery.
To elucidate the relation of changes in computerized vectorcardiographic trend parameters indicating perioperative myocardial ischemia with perioperative cardiac complications. ⋯ Vectorcardiographic signs of myocardial ischemia were significantly increased intraoperatively, but most pronounced postoperatively in the patients subsequently suffering cardiac events. The changes could be related to the individual cardiac morbidity with acceptable precision. Thus, continuous vectorcardiographic monitoring may be beneficial for patients at risk of developing perioperative ischemia.

Comparative StudyNew equipment for neuromuscular transmission monitoring: a comparison of the TOF-Guard with the Myograph 2000.
The present study is to clarify whether the bias and limits of agreement of the TOF-Guard and the mechanomyograph differ from those of two mechanomyographs on contra lateral arms. Previous studies of the bias and limits of agreement between acceleromyographical (TOF-Guard) and mechanomyographical measurements of neuromuscular transmission did not take the error introduced by using contra lateral arms into consideration. ⋯ Due to wide limits of agreement and different recovery courses, acccleromyographic and mechanomyographic recordings of neuromuscular transmission cannot be used interchangeably. The substantial variation between simultaneous mechanomyographical recordings of neuromuscular transmission obtained in contra lateral arms suggests that this factor should be taken into account when studying new neuromuscular monitoring techniques using the two-arm technique.
