Comparative StudyAn avoidable methodological failure in intracranial pressure monitoring using fiberoptic or solid state devices.
Failure of intraventricular pressure (IVP) measurement in case of catheter blockage is believed to be eliminated by using intraventricular microtransducers. We report about an avoidable methodological error, which may affect the reliability of IVP measurement with these devices. Intraventricular fiberoptic or solid state devices were implanted in 43 patients considered to be at risk for catheter occlusion. ⋯ In patients treated with Type B devices, no erroneous pressure recording could be identified, irrespective if CSF drainage was performed or not. Transducers, which are simply placed inside the ventriculostomy catheter require fluid coupling. They may fail, either during CSF drainage or when the catheter is blocked or placed within the parenchyma.

Near-infrared reflected spectroscopy and electroencephalography during carotid endarterectomy--in search of a new shunt criterion.
The aim of this clinical study was to evaluate cerebral oximetry with near-infrared reflected spectroscopy (NIRS) as a monitoring system during carotid endarterectomy. The cross-clamping changes of cerebrovascular hemoglobin oxygen saturation (cereb. O2 satn.) were compared with data from a processed EEG analysis. ⋯ Higher cut-off values resulted in a gradual increase of the specificity at the expense of a significant decrease of the sensitivity. In conclusion, improved validation and calibration techniques are necessary before this technique may be used for relevant assessment of cerebral oxygenation during carotid surgery. In particular, in order to define a new shunt criterion, the focal aspect of this new technique is probably one of the limitations.

Diagnostics of spontaneous cough in childhood asthma: results of continuous tracheal sound recording in the homes of children.
Spontaneous cough is a symptom with diffuse diagnostic significance for childhood asthma. Information on the interrelations between presence/frequency of cough and other symptoms of asthma are not available to the physician. Tracheal sounds were continuously recorded in the homes of 60 children with and 30 without asthma, at 72 and 24 h, respectively. ⋯ Three children did not cough during exacerbations. Persistent, isolated cough was observed in two children. It was concluded that spontaneous cough is modestly predictive of asthmatic exacerbations, but not of a diagnosis of asthma or severity of asthma.

Complications and safety associated with ICP monitoring: a study of 542 patients.
In our institution ICP was monitored in patients with GCS < or = 8 and abnormal CT scan: 362 severely head injured and 180 subarachnoid hemorrhage. Mean duration of monitoring was 103.6 hours (SD 74.96). Among 542 patients, 440 showed at least one episode of ICP above the threshold of 20 mm Hg. ⋯ In 13 cases (2.2%) a ventricular infection has been diagnosed. In 1 case an intraparenchimal hemorrhage related to the presence of the catheter was detected. Elevated risk of HICP and low incidence of complications have been shown in this series.

Comparative Study Clinical TrialClinical evaluation of the on-line Sensicath blood gas monitoring system.
Blood gas analysis is an integral part of the management of open heart surgery and post-surgical intensive care patients. This study was conducted to compare the results of on-line blood gas analysis using the new Sensicath (SC) optical sensor technology against standard blood gas assay. ⋯ In summary, this study found the Sensicath to be an accurate, useful tool which permits immediate blood gas analysis without blood exposure and constitutes an advance in the field of intensive care monitoring.
